氢氧化钙糊剂联合根管治疗术治疗根尖炎的临床价值分析

Clinical Value Analysis of Calcium Hydroxide Paste Combined with Root Canal Therapy in the Treatment of Apical Periodontitis

目的探究临床根尖炎治疗中联合应用氢氧化钙糊剂与根管治疗术的效果。方法选取洛阳市第三人民医院口腔科2021年1月至2023年12月期间接收治疗的80例根尖炎患者,依据乱数表法,将其分为研究组(n=40,氢氧化钙糊剂+根管治疗)、对照组(n=40,根管治疗),比较两组疗效、炎症指标、牙周指标、临床改善指标及生活质量的差异。结果研究组治疗有效率(100.00%)高于对照组(87.50%),差异具有统计学意义(P<0.05);治疗前,两组降钙素原(PCT)、C反应蛋白(CRP)、白细胞计数(WBC)水平比较,差异无统计学意义(P>0.05),治疗2周后,两组PCT、CRP、WBC水平均低于治疗前,且研究组低于对照组,差异均有统计学意义(P<0.05);治疗前,两组牙龈指数(GI)、菌斑指数(PLI)、龈沟出血指数(SBI)水平比较,差异无统计学意义(P>0.05),治疗2周后,两组GI、PLI、SBI水平均降低,且研究组低于对照组,差异均有统计学意义(P<0.05);治疗前,两组疼痛数字评定量表(NRS)、改良曼恩咀嚼能力评估量表(MASA)评分差异无统计学意义(P>0.05),治疗2周后,两组疼痛NRS评分降低、MASA评分提高,且研究组疼痛NRS评分低于对照组、MASA评分高于对照组,差异均有统计学意义(P<0.05);治疗前,两组简化牙本质敏感影响程度量表(DHEQ-15)各维度评分差异无统计学意义(P>0.05),治疗2周后,两组DHEQ-15各维度评分均低于治疗前,且研究组低于对照组,差异均有统计学意义(P<0.05)。结论联合应用氢氧化钙糊剂与根管治疗术治疗根尖炎,能够增强根管治疗的临床疗效,减少炎症反应,促进牙周指标恢复速度,改善患者临床疼痛感、自身咀嚼功能以及生活质量,值得临床应用。

Objective To explore the clinical effect of combining calcium hydroxide paste with root canal therapy in the treatment of apical periodontitis.Methods A total of 80 patients with apical periodontitis treated at the Department of Stomatology of Luoyang Third People's Hospital from January 2021 to December 2023 were selected and divided into a study group(n=40,treated with calcium hydroxide paste combined with root canal therapy)and a control group(n=40,treated with root canal therapy only).according to the random number table method.The efficacy,inflammatory indicators,periodontal indicators,clinical improvement indicators,and quality of life differences between the two groups were compared.Results The treatment efficacy rate of the study group(100.00%)was higher than that of the control group(87.50%),and the difference was statistically significant(P<0.05);Before treatment,there was no statistically significant difference in the levels of procalcitonin(PCT),C-reactive protein(CRP),and white blood cell count(WBC)between the two groups(P>0.05).After 2 weeks of treatment,the levels of PCT,CRP,and WBC in both groups were lower than before treatment,and the study group was lower than the control group,with statistically significant differences(P<0.05);Before treatment,there was no statistically significant difference in the levels of gingival index(GI),plaque index(PLI),and sulcus bleeding index(SBI)between the two groups(P>0.05).After 2 weeks of treatment,the levels of GI,PLI,and SBI in both groups decreased,and the study group was lower than the control group,with statistically significant differences(P<0.05);Before treatment,there was no statistically significant difference in the scores of the Pain Numeric Rating Scale(NRS)and the modified Mann Assessment of Mastication Ability(MASA)between the two groups(P>0.05).After 2 weeks of treatment,the NRS scores of the two groups decreased while the MASA scores increased.Moreover,the NRS scores of the study group were lower than those of the control group,while the MASA scores were higher than those of the control group,and the differences were statistically significant(P<0.05);Before treatment,there was no statistically significant difference in the scores of each dimension of the Dentine Hypersensitivity Experience Questionnaire(DHEQ-15)between the two groups(P>0.05).After 2 weeks of treatment,the scores of each dimension of DHEQ-15 in both groups were lower than before treatment,and the study group was lower than the control group,with statistically significant differences(P<0.05).Conclusion The combination of calcium hydroxide paste and root canal therapy enhances clinical efficacy,reduces inflammatory responses,promotes periodontal recovery,improves clinical pain perception,masticatory function,and quality of life in patients with apical periodontitis,making it worthy of clinical application.
