定眩汤联合针灸治疗眩晕疗效观察

Observation of the Efficacy of Dingxuan Decoction Combined with Acupuncture in the Treatment of Vertigo

目的探析定眩汤联合针灸治疗眩晕效果。方法选取2021年2月至2023年2月期间在周口市中医院治疗的86例眩晕患者,以随机数表法将其均分为两组,对照组(接受针灸治疗)及观察组(接受定眩汤联合针灸治疗)各43例,对比两组中医证候积分、血液流变学[血浆黏度、高切全血黏度、低切全血黏度]、血流动力学[椎、基底动脉搏动指数(PI)、平均血流速度(Vm)]、氧化应激指标[超氧化物歧化酶(SOD)、丙二醛(MDA)、一氧化氮(NO)]、血管内皮功能[内皮素-1(ET-1)、血管性血友病因子(vWF)、血栓调节蛋白(TM)]。结果治疗后,两组眩晕耳鸣、心悸气短、面色苍白、倦怠乏力评分低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。治疗后,两组血浆黏度、高切全血黏度、低切全血黏度指标低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。治疗后,两组基底动脉、椎动脉PI较治疗前慢、Vm较治疗前快,且观察组PI慢于对照组、Vm快于对照组,差异有统计学意义(P<0.05)。治疗后,两组SOD、NO水平高于治疗前、MDA水平低于治疗前,且观察组SOD、NO水平高于对照组,MDA水平低于对照组,差异有统计学意义(P<0.05)。治疗后,两组ET-1、vWF、TM水平低于治疗前,且观察组ET-1、vWF、TM水平低于对照组,差异有统计学意义(P<0.05)。结论眩晕患者采用定眩汤联合针灸治疗,可减轻患者眩晕症状,改善血液黏度、血流动力学,降低氧化应激反应,调节血管内皮功能。

Objective To explore the efficacy of Dingxuan Decoction combined with acupuncture in treating vertigo.Methods 86 patients with vertigo treated at Zhoukou Hospital of Traditional Chinese Medicine from February 2021 to February 2023 were selected and randomly divided into two groups:a control group(43 patients,receiving acupuncture)and an observation group(43 patients,receiving Dingxuan Decoction combined with acupuncture).The groups were compared based on TCM syndrome scores,hemorheology indices(plasma viscosity,high-shear whole blood viscosity,low-shear whole blood viscosity),hemodynamics[vertebral and basilar artery pulsatility index(PI),mean velocity(Vm)],oxidative stress indicators[superoxide dismutase(SOD),malondialdehyde(MDA),nitric oxide(NO)],and endothelial function indicators[endothelin-1(ET-1),von Willebrand factor(vWF),thrombomodulin(TM)].Results After treatment,the scores of vertigo,tinnitus,palpitations,shortness of breath,pale complexion,fatigue and weakness in both groups were lower than before treatment,and the observation group was lower than the control group,the difference was statistically significant(P<0.05).After treatment,the plasma viscosity,high shear whole blood viscosity,and low shear whole blood viscosity indicators in both groups were lower than before treatment,and the observation group was lower than the control group,the difference was statistically significant(P<0.05).After treatment,the PI of the basilar artery and vertebral artery in both groups was slower than before treatment,and the Vm was faster before treatment.Moreover,the PI in the observation group was slower than in the control group,and the Vm was faster than that in the control group,the difference was statistically significant(P<0.05).After treatment,the levels of SOD and NO in both groups were higher than before treatment,and the level of MDA was lower than before treatment.The observation group had higher levels of SOD and NO than those in the control group,and lower levels of MDA than the control group,the difference was statistically significant(P<0.05).After treatment,the levels of ET-1,vWF,and TM in both groups were lower than before treatment,and the levels of ET-1,vWF,and TM in the observation group were lower than those in the control group,the difference was statistically significant(P<0.05)..Conclusion The combination of Dingxuan Decoction and acupuncture can effectively alleviate vertigo symptoms,improve blood viscosity,hemodynamics,reduce oxidative stress,and regulate endothelial function in patients with vertigo.
