常规超声联合弹性成像超声在甲状腺乳头状癌诊断中的应用分析

Application Analysis of Conventional Ultrasound Combined with Elastography in the Diagnosis of Papillary Thyroid Carcinoma

目的研究甲状腺乳头状癌在临床诊断中常规超声结合弹性成像超声的应用及其诊断价值。方法抽取2023年1月至2023年12月郑州大学第二附属医院收入的120例疑似甲状腺乳头状癌患者进行回顾性分析,所有患者均开展常规超声、弹性成像超声检查,以病理诊断结果作为“金标准”,分析不同检查方式对甲状腺乳头状癌的诊断价值。结果120例患者经病理检查显示甲状腺乳头状癌72例,良性甲状腺结节48例;常规超声检出58例,弹性成像超声检出65例,常规+弹性成像超声检出70例,常规+弹性成像超声诊断敏感性、准确性均高于单一常规超声、弹性超声检查,差异均有统计学意义(P<0.05),但特异性相比,差异无统计学意义(P>0.05);常规+弹性超声约登指数最高(0.903)。结论联合应用常规+弹性成像超声诊断甲状腺乳头状癌相较于单一使用常规超声或弹性超声检查,其具有更高的临床应用价值,两种检查方式相互补充,可为临床医师提供参考,提高诊断准确性。

Objective To study the application and diagnostic value of conventional ultrasound combined with elastography in the clinical diagnosis of papillary thyroid carcinoma.Methods A retrospective analysis was conducted on 120 patients with suspected papillary thyroid carcinoma who were admitted to the Second Affiliated Hospital of Zhengzhou University from January 2023 to December 2023.All patients underwent conventional ultrasound and elastography,with pathological diagnosis results used as the"gold standard"to analyze the diagnostic value of different examination methods for papillary thyroid carcinoma.Results Among the 120 patients,72 were confirmed to have papillary thyroid carcinoma,and 48 had benign thyroid nodules through pathological examination.Conventional ultrasound detected 58 cases,elastography detected 65 cases,and the combination of conventional ultrasound and elastography detected 70 cases.The sensitivity and accuracy of combined conventional ultrasound and elastography were higher than those of single conventional ultrasound or elastography alone,with statistically significant differences(P<0.05).However,there was no statistically significant difference in specificity(P>0.05).The Youden index was highest for the combined conventional and elastography ultrasound(0.903).Conclusion For patients with papillary thyroid carcinoma,the combined use of conventional ultrasound and elastography has higher clinical application value compared to either method alone.The two examination methods complement each other and can provide valuable references for clinicians,improving diagnostic accuracy.
