^(18)F-FDGPET-CT参数预测肺腺癌EGFR突变的价值分析

摘要 目的探讨18氟-脱氧葡萄糖正电子发射计算机断层成像(^(18)F-FDG PET-CT)参数预测表皮生长因子受体(EGFR)突变的价值。方法选取南阳胸科医院2021年1月至2023年6月收治的100例肺腺癌患者,根据EGFR基因突变检测分为EGFR突变型(n=42)和EGFR野生型(n=58),所有患者均进行^(18)F-FDG PET-CT显像检查。分析EGFR突变型与EGFR野生型临床资料及最大标准化摄取值(SUVmax)、糖酵解总量(TLG)、代谢肿瘤体积(MTV)水平,分析影响肺腺癌患者EGFR突变的因素,受试者工作特征(ROC)曲线分析SUVmax、TLG、MTV预测肺腺癌患者EGFR突变的价值。结果EGFR突变型患者女性占比高于EGFR野生型患者,差异具有统计学意义(P<0.05),EGFR突变型患者肿瘤直径、吸烟史占比、SUVmax、TLG、MTV水平低于EGFR野生型患者,差异均有统计学意义(P<0.05);SUVmax、TLG、MTV均为影响肺腺癌患者EGFR突变因素,差异均有统计学意义(P<0.05);SUVmax预测肺腺癌患者EGFR突变的曲线下面积(AUC)值、敏感度、特异度分别为0.913、87.93%、90.48%,TLG预测肺腺癌患者EGFR突变的AUC值、敏感度、特异度分别为0.740、81.03%、66.67%,MTV预测肺腺癌患者EGFR突变的AUC值、敏感度、特异度分别为0.815、81.03%、73.81%。结论^(18)F-FDG PET-CT参数中的SUVmax、TLG、MTV水平可为预测肺腺癌EGFR突变提供价值。 Objective To explore the value of ^(18)F-fluorodeoxyglucose positron emission tomography-computed tomography(^(18)F-FDG PET-CT)parameters in predicting epidermal growth factor receptor(EGFR)mutations.Methods A total of 100 patients with lung adenocarcinoma treated at Nanyang Thoracic Hospital from January 2021 to June 2023 were selected.Based on EGFR gene mutation testing,patients were divided into EGFR mutation group(n=42)and EGFR wild-type group(n=58).All patients underwent ^(18)F-FDG PET-CT imaging.Clinical data,maximum standardized uptake value(SUVmax),total lesion glycolysis(TLG),and metabolic tumor volume(MTV)levels were analyzed between the EGFR mutation and wild-type groups.Factors affecting EGFR mutations in lung adenocarcinoma patients were analyzed,and the value of SUVmax,TLG,and MTV in predicting EGFR mutations was evaluated using receiver operating characteristic(ROC)curve analysis.Results The proportion of females in the EGFR mutation group was higher than that in the EGFR wild-type group,with statistically significant differences(P<0.05).Tumor diameter,smoking history,SUVmax,TLG,and MTV levels in the EGFR mutation group were lower than those in the EGFR wild-type group,with statistically significant differences(P<0.05).SUVmax,TLG,and MTV were factors influencing EGFR mutations in lung adenocarcinoma patients,with statistically significant differences(P<0.05).The area under the ROC curve(AUC),sensitivity,and specificity of SUVmax for predicting EGFR mutations were 0.913,87.93%,and 90.48%,respectively;for TLG,they were 0.740,81.03%,and 66.67%,respectively;for MTV,they were 0.815,81.03%,and 73.81%,respectively.Conclusion The levels of SUVmax,TLG,and MTV in ^(18)F-FDG PET-CT parameters can provide valuable information for predicting EGFR mutations in lung adenocarcinoma.
