高频超声联合弹性成像对于上肢神经卡压综合征的诊断价值探析

摘要 目的:探讨高频超声联合弹性成像对于上肢神经卡压综合征的诊断价值。方法:选择2021年1月~2022年12月来本院进行检查的132例作为本次研究对象。在此期间选取66例确诊为上肢神经卡压综合症的患者作为试验组,并选取同期的66例健康人群作为对照组,且试验组依照患者病情的严重程度,将其分为轻症组与中重症组两组。所有受试者均行常规超声及剪切波弹性成像检查,在豌豆骨及肘管入口水平分别测量正中神经及尺神经前后径、左右经、横截面面积、杨氏模量。比较各个测量指标在各组间的差异及高频超声联合弹性成像的临床分型价值。结果:对比观察与对照两组检查结果发现,试验组杨氏模量及正中神经横截面积高于对照组,差异具有统计学意义(P<0.05);对比观察与对照两组检查结果发现,试验组横截面积、尺神经前后径、杨氏模量高于对照组,差异具有统计学意义(P<0.05);对比观察与对照两组检查结果发现,轻症组正中神经杨氏模量低于中重症组,差异具有统计学意义(P<0.05);对比观察与对照两组检查结果发现,轻症组尺神经杨氏模量低于中重症组,差异具有统计学意义(P<0.05);高频超声联合弹性成像的诊断效能(准确度、精确度、特异度、敏感度)较单一高频超声高。结论:高频超声联合弹性成像表现出较好的诊断价值。 Objective:To evaluate the diagnostic value of high-frequency ultrasound combined with elastic imaging in upper limb nerve entrapment syndrome.Methods:132 people who came to our hospital for examination from January 2021 to December 2022 were selected as the subjects of this study.During this period,66 patients diagnosed with upper limb nerve entrapment syndrome were selected as experimental group,and 66 healthy people were selected during the same period as control group,and experimental group was divided into mild group and severe group according to the severity of the patients’conditions.All subjects underwent conventional ultrasonography and shear-wave elastography.The anterior-posterior diameter,left-right meridian,cross-sectional area and Young’s modulus of median nerve and ulnar nerve were measured at the entrance level of pea bone and elbow canal,respectively.To compare the difference of each measurement index in each group and the clinical classification value of high-frequency ultrasound combined with elastic imaging.Results:It was found that the Young’s modulus and median nerve cross-sectional area in experimental group were higher than those in control group,and the difference was statistically significant(P<0.05).It was found that the cross-sectional area,anterior and posterior diameter of ulnar nerve and Young’s modulus in experimental group were higher than those in control group,and the differences were statistically significant(P<0.05).The results of comparison between the two groups showed that the median nerve Young’s modulus in the mild group was lower than that in the moderate and severe group,and the difference was statistically significant(P<0.05).The results of comparison observation and control group showed that the Young’s modulus of ulnar nerve in mild disease group was lower than that in moderate and severe disease group,and the difference was statistically significant(P<0.05).The diagnostic efficiency(accuracy,precision,specificity,sensitivity)of highfrequency ultrasound combined with elastic imaging is higher than that of single high-frequency ultrasound.Conclusion:High-frequency ultrasound combined with elastic imaging shows good diagnostic value.
