卒中相关性肺炎患者实施床旁纤维支气管镜吸痰及肺泡灌洗治疗的疗效和安全性

Efficacy and Safety of Sputum Aspiration and Alveolar Lavage in Patients with Stroke-Associated Pneumonia Treated by Fiberoptic Bronchoscopy

目的:探讨分析卒中相关性肺炎患者实施床旁纤维支气管镜吸痰及肺泡灌洗治疗的疗效和安全性。方法:本次研究病例治疗时间为2020年12月~2022年1月,研究对象选择方式为电脑随机抽选,均为本院接受治疗的患有卒中相关性肺炎患者,根据治疗方式差异分为常规治疗和使用床旁纤维支气管镜吸痰及肺泡灌洗治疗,分别对应对照组与观察组,每组各77例患者,共计154例符合条件患者,对研究完成后结果进行比较与分析。结果:获得更高临床疗效的为观察组(P<0.05);住院、抗生素使用时间以及每天吸痰次数更多的为对照组(P<0.05);发生不良反应事件概率更高的为对照组(P<0.05);各项肺功能指标在接受治疗前两组之间均无差异(P>0.05),在接受治疗后各项指标之中数值更高的为观察组(P<0.05)。结论:对于患有卒中相关性肺炎的患者而言,对其通过使用床旁纤维支气管镜吸痰及肺泡灌洗的方式进行治疗,让患者获得了更加接近理想状态的治疗成效,患者能够拥有更高的治疗安全性,肺功能得到了较大程度的改善,是一种值得临床推广与使用的方式。

Objective:To investigate and analyze the efficacy and safety of sputum aspiration and alveolar lavage in patients with stroke-associated pneumonia.Methods:Patients in this study were treated from December 2020 to January 2022,and the study subjects were randomly selected by computer.They were all patients with stroke-related pneumonia who received treatment in our hospital.According to the differences in treatment methods,they were divided into conventional treatment,sputum aspiration with bedside fiberbronchoscope and alveolar lavage,corresponding to the control group and the observation group,respectively.There were 77 patients in each group,a total of 154 eligible patients,and the results were compared and analyzed after the study was completed.Results:Observation group was the group with higher clinical effect(P<0.05).The group with more hospitalization,antibiotic use time and sputum aspiration times per day was the control group(P<0.05).The group with higher probability of adverse events was the control group(P<0.05).There was no difference in the pulmonary function index between the two groups before treatment(P>0.05),and the group with higher value after treatment was the observation group(P<0.05).Conclusion:For patients with stroke-associated pneumonia,the treatment of sputum aspiration and alveolar lavage by bedside fiberoptic bronchoscope can bring the treatment effect closer to the ideal state,the patients can have higher treatment safety,and the lung function has been greatly improved,which is a method worthy of clinical practical promotion and use.
