心脏彩超对高血压左心室肥厚伴左心衰竭的临床诊断意义

The Diagnostic Significance of Echocardiography in Hypertensive Left Ventricular Hypertrophy with Left Heart Failure

目的:研究高血压左心室肥厚伴左心衰竭的心脏超声诊断效果。方法:选择所在中医院2021年5月~2023年5月彩超室诊断为高血压左心室肥厚伴左心衰竭患者作为此次研究的观察组(66例),另取同期体检人员为对照组(66例),对比分析两组血压[包括收缩压(SBP)、舒张压(DBP)]、心脏彩超检查结果[包括左房内径(LAD)、左室舒张末期内径(LVEDD)、左室收缩末期内径(LVESD)、左室射血分数(LVEF)、左室短轴缩短率(LVFS)、二尖瓣血流舒张早期最大速率(E峰)、舒张晚期心房收缩最大峰值速率(A峰)及二尖瓣舒张早期峰值速率/舒张晚期峰值速率(E/A)]。结果:①观察组患者SBP、DBP、LVFS及A峰高于对照组,而LAD、LVESD及LVEDD均长于对照组,LVEF、E峰及E/A均低于对照组,数据之间差异显著(P<0.05);②处于不同心功能等级患者血压、彩超检查各指标结果差异明显(P<0.05),且随着心功能分级升高,患者SBP、DBP、LVFS及A峰逐渐升高,LAD、LVESD及LVEDD则逐渐延长,而LVEF、E峰及E/A逐渐降低,各分级患者数据之间差异显著(P<0.05)。结论:对于高血压左心室肥厚伴左心衰患者通过借助心脏彩超进行诊断,可以准确有效判断患者病情及心功能情况,为临床治疗提供重要参考依据。

Objective:Study the diagnostic effect of cardiac ultrasound on hypertensive left ventricular hypertrophy with left heart failure.Methods:During the period from May 2021 to May 2023 at the Traditional Chinese Medicine Hospital where the selection hospital is located,patients diagnosed with hypertension,left ventricular hypertrophy,and left heart failure by color Doppler ultrasound were selected as the observation group(66 cases)for this study,while those who underwent physical examination at the same time were selected as the control group(66 cases).Comparative analysis of blood pressure between two groups[including systolic blood pressure(SBP),diastolic blood pressure(DBP)],and results of cardiac ultrasound examination[including left atrial diameter(LAD),left ventricular end diastolic diameter(LVEDD),left ventricular end systolic diameter(LVESD),left ventricular ejection fraction(LVEF),left ventricular short axis shortening rate(LVFS),mitral valve blood flow maximum velocity in early diastole(E peak),atrial systolic maximum peak velocity in late diastole(A peak),and mitral valve peak velocity in early diastole/late diastole(E/A)].Results:①SBP,DBP,LVFS and peak A in observation group were higher than those in control group,while LAD,LVESD and LVEDD were longer than those in control group,LVEF,peak E and E/A were lower than those in control group,the difference was significant(P<0.05);②The results of blood pressure and color ultrasound in patients with different cardiac function grades were significantly different(P<0.05).With the increase of cardiac function grades,SBP,DBP,LVFS and peak A gradually increased,LAD,LVESD and LVEDD gradually prolonged,while LVEF,peak E and E/A gradually decreased.There was a significant difference in the data among different grades(P<0.05).Conclusion:Patients with hypertensive left ventricular hypertrophy and left heart failure can accurately and effectively assess their condition and cardiac function by using cardiac ultrasound for diagnosis,provide an important reference for clinical treatment.
