高龄妊娠晚期孕妇心理压力现状及其影响因素

Status of psychological stress and its influencing factors in elderly pregnant women in late pregnancy

目的:分析高龄妊娠晚期孕妇心理压力现状及其影响因素。方法:选取2023年1—10月于该院进行产检的189名高龄妊娠晚期孕妇进行横断面研究,收集孕妇一般资料,采用妊娠压力量表(PSRS)评估其心理压力,采用多元线性回归分析高龄妊娠晚期孕妇心理压力的影响因素。结果:189名高龄妊娠晚期孕妇平均PSRS评分为(43.40±6.18)分;不同年龄、流产史、孕妇或配偶为独生子女、按时产检、文化程度、在职、居住地区、计划妊娠、家庭人均月收入的高龄妊娠晚期孕妇PSRS评分比较,差异均无统计学意义(P>0.05);孕周<37周、初产、有妊娠期并发症、家庭功能障碍、心理弹性低的高龄妊娠晚期孕妇PSRS评分均高于其他类型孕妇,差异有统计学意义(P<0.05);多元线性回归分析结果显示,孕周<37周、初产、有妊娠期并发症、家庭功能障碍、心理弹性低均为高龄妊娠晚期孕妇心理压力大的危险因素(P<0.05)。结论:孕周<37周、初产、有妊娠期并发症、家庭功能障碍、心理弹性低均为高龄妊娠晚期孕妇心理压力大的危险因素。

Objective:To analyze the status of psychological stress and its influencing factors in elderly pregnant women in late pregnancy.Methods:A cross-sectional study was conducted on 189 elderly pregnant women who underwent prenatal examination in the hospital from January to October 2023.The general information of these pregnant women was collected.The pregnancy stress rating scale(PSRS)was used to evaluate the psychological stress of these pregnant women.Multiple linear regression was used to analyze the influencing factors of psychological stress in these elderly pregnant women in late pregnancy.Results:The average PSRS score of the 189 pregnant women elderly pregnant women in late pregnancy was(43.40±6.18)points.There were no significant differences in the PSRS scores among the pregnant women of different ages,abortion history,pregnant women or spouses as the only child,on-time prenatal examination,education level,on-the-job,living area,planned pregnancy,and family monthly income per capita(P>0.05).The PSRS scores of the elderly pregnant women with gestational age<37 weeks,primiparity,pregnancy complications,family dysfunction and low psychological resilience were higher than those of other types of pregnant women,and the differences were statistically significant(P<0.05).Multiple linear regression analysis showed that gestational age<37 weeks,primiparity,pregnancy complications,family dysfunction,and low psychological resilience were all risk factors for psychological stress in the elderly pregnant women in late pregnancy(P<0.05).Conclusions:Gestational age<37 weeks,primiparity,pregnancy complications,family dysfunction,and low psychological resilience are the risk factors for psychological stress in the elderly pregnant women in late pregnancy.
