炎症因子与过敏性紫癜患儿肾功能损伤的相关性分析

Correlation between inflammatory factors and renal function injury in children with Henoch-Schönlein purpura

摘要 目的探讨过敏性紫癜患儿血清骨桥蛋白、免疫球蛋白A1(IgA1)、基质金属蛋白酶-9(MMP-9)、核转录因子-κB(NF-κB)水平表达及其同肾功能损伤的相关性。方法本研究为病例对照研究,选取2020年11月至2023年11月西安交通大学第一附属医院皮肤科收治的120例过敏性紫癜患儿作为过敏性紫癜组,男64例,女56例,年龄(4.00±1.35)岁,年龄范围为3~6岁;另选取同期体检的84名健康小儿作为健康组,男49名,女35名,年龄(3.85±1.44)岁,年龄范围为3~6岁。比较两组纳入者血清骨桥蛋白、IgA1、MMP-9、NF-κB水平表达;分析血清骨桥蛋白、IgA1、MMP-9、NF-κB水平对肾功能损伤的预测价值。结果过敏性紫癜组血清骨桥蛋白[(94.33±21.84)pg/ml]、MMP-9[(160.44±87.33)μg/L]及NF-κB[(51.34±16.28)μmol/L]水平高于健康组[(35.11±11.34)pg/ml、(22.35±11.25)μg/L、(18.47±8.66)μmol/L],IgA1[(1.54±0.66)g/L]水平低于健康组[(2.48±0.34)g/L];过敏性紫癜组中,出现肾损伤的患儿50例,未出现肾损伤的患儿70例,肾损伤患儿血清骨桥蛋白[(114.33±24.65)pg/ml]、MMP-9[(174.46±74.11)μg/L]、NF-κB[(54.33±15.85)μmol/L]水平高于无肾损伤患儿[(87.33±14.22)pg/ml、(124.56±61.22)μg/L、(44.13±11.05)μmol/L]、IgA1水平[(1.13±0.11)g/L]低于无肾损伤患儿[(1.72±0.32)g/L],差异有统计学意义(P<0.05)。相关性分析显示,肾功能损伤与血清骨桥蛋白(r=1.24)、MMP-9(r=1.17)及NF-κB(r=1.31)呈正相关,与IgA1(r=-1.07)呈负相关(P<0.05)。结论过敏性紫癜患儿血清骨桥蛋白、MMP-9、NF-κB高于健康小儿,IgA1水平低于健康小儿;肾功能损伤与血清骨桥蛋白、MMP-9及NF-κB呈正相关;与IgA1呈负相关。 Objective To investigate the levels of serum osteopontin,immunoglobulin A1(IgA1),matrix metalloproteinase-9(MMP-9)and nuclear transcription factor-κB(NF-κB)in children with Henoch-Schönlein purpura and their correlation with renal function injury.Methods This study was a case-control study,a total of 120 children with Henoch-Schönlein purpura were selected from the department of Dermatology of The First Affiliated Hospital of Xi′an Jiaotong University from November 2020 to November 2023 as Henoch-Schönlein purpura group,including 64 males and 56 females,aged(4.00±1.35)years old,ranging from 3 to 6 years old.Another 84 healthy children in the same period of physical examination were selected as healthy group,including 49 males and 35 females,aged(3.85±1.44)years old,ranging from 3 to 6 years old.The levels of serum osteopontin,IgA1,MMP-9 and NF-κB were compared between the two groups,and the predictive value of the levels of osteopontin,IgA1,MMP-9 and NF-κB on renal function injury was analyzed.Results In Henoch-Schönlein purpura group,serum osteopontin[(94.33±21.84)pg/ml],MMP-9[(160.44±87.33)μg/L]and NF-κB[(51.34±16.28)μmol/L]levels were higher than those in healthy group[(35.11±11.34)pg/ml,(22.35±11.25)μg/L,(18.47±8.66)μmol/L],and IgA1[(1.54±0.66)g/L]levels were lower than those in healthy group[(2.48±0.34)g/L];in Henoch-Schönlein purpura group,there were 50 children had renal injury and 70 children had no renal injury,serum osteopontin[(114.33±24.65)pg/ml],MMP-9[(174.46±74.11)μg/L],and NF-κB[(54.33±15.85)μmol/L]levels were higher in children with renal injury than in those without renal injury[(87.33±14.22)pg/ml,(124.56±61.22)μg/L,(44.13±11.05)μmol/L],and IgA1 levels[(1.13±0.11)g/L]were lower than in those without renal injury[(1.72±0.32)g/L],the differences were statistically significant(P<0.05).Correlation analysis showed that renal function injury was positively correlated with serum osteopontin(r=1.24),MMP-9(r=1.17)and NF-κB(r=1.31);and negatively correlated with IgA1(r=-1.07,P<0.05).Conclusions Serum osteopontin,MMP-9 and NF-κB levels were higher and IgA1 levels were lower in children with Henoch-Schönlein purpura than in healthy children;renal function injury was positively correlated with serum osteopontin,MMP-9 and NF-κB;and negatively correlated with IgA1.
