不同剂量黄体酮辅治无排卵型月经失调的效果及对患者经期、子宫内膜厚度的影响

Effect of Different Doses of Progesterone in the Adjuvant Treatment of An⁃ovulatory Menstrual Disorders and its Influence on Menstrual Period and Endometrial Thickness of Patients

目的研讨不同剂量黄体酮辅治无排卵型月经失调的效果及对患者经期、子宫内膜厚度的影响。方法随机选取2020年1月—2023年12月福建省漳州市平和县医院收治的100例无排卵型月经失调患者为研究对象,根据不同用药剂量分为对照组和观察组,各50例。对照组接受高剂量(200 mg/d)黄体酮辅治,观察组接受低剂量(100 mg/d)黄体酮辅治,对比两组患者不同治疗阶段的经期改善状况、子宫内膜厚度、激素水平以及用药安全性。结果治疗后,两组患者的经期改善状况(出血量得分、月经周期)、子宫内膜厚度以及激素指标(雌二醇、促黄体生成素与促卵泡激素)对比,差异无统计学意义(P均>0.05)。观察组用药不良反应总发生率4.00%(2/50)低于对照组的18.00%(9/50),差异有统计学意义(χ^(2)=5.005,P=0.025)。结论低剂量(100 mg/d)黄体酮辅治无排卵型月经失调,对患者经期、子宫内膜厚度及激素水平的改善效果与高剂量(200 mg/d)效果相当,同时还可减少用药不良反应发生,提升用药安全性。

Objective To study the effect of different doses of progesterone in the treatment of anovulatory menstrual disorders and its effect on menstrual period and endometrial thickness.Methods A total of 100 patients with anovula⁃tory menstrual disorders admitted to Zhangzhou Pinghe County Hospital,Fujian Province from January 2020 to De⁃cember 2023 were randomly selected and divided into control group and observation group according to different doses,with 50 cases in each group.The control group received high-dose(200 mg/d)progesterone adjuvant therapy,and the observation group received low-dose(100 mg/d)progesterone adjuvant therapy.The menstrual improvement,endometrial thickness,hormone level and drug safety of the two groups were compared at different treatment stages.Results After treatment,there was no significant difference in menstrual improvement(bleeding volume score,men⁃strual cycle),endometrial thickness and hormone indexes(estradiol,luteinizing hormone and follicle stimulating hor⁃mone)between the two groups(all P>0.05).The total incidence of adverse drug reactions in the observation group 4.00%(2/50)was lower than that in the control group 18.00%(9/50),and the difference was statistically significant(χ^(2)=5.005,P=0.025).Conclusion Low dose(100 mg/d)of progesterone in the treatment of anovulatory menstrual disor⁃ders has the same effect on the improvement of menstrual period,endometrial thickness and hormone level as high dose(200 mg/d).At the same time,it can reduce the occurrence of adverse drug reactions and improve the safety of medication.
