间歇充气加压泵最佳治疗时间对危重症患者深静脉血栓的影响

The Effect of Optimal Treatment Time for Intermittent Pneumatic Com⁃pression Pump on Deep Vein Thrombosis in Critically Ill Patients

摘要 目的探讨针对临床危重患者,采取间歇充气加压泵不同的护理使用时间对患者深静脉血栓的影响。方法回顾性选取2021年6月—2023年6月巨野县人民医院重症医学科收治的160例危重症患者的临床资料,按照间歇充气加压泵不同的护理使用时间分为两组,每组80例,对照组间歇充气加压泵使用时间为20 min,2次/d,观察组为40 min,2次/d。比较两组深静脉血栓发生率、软组织损伤情况、股静脉血流动力学指标、护理满意度。结果观察组深静脉血栓发生率为2.50%(2/80),低于对照组的11.25%(9/80),差异有统计学意义(χ^(2)=4.783,P=0.029)。观察组组织损伤发生率为2.50%(2/80),对照组为1.25%(1/80),两组比较,差异无统计学意义(χ^(2)=0.000,P=1.000)。护理后,观察组血流峰速度、血流平均速度、血流量均高于对照组,差异有统计学意义(P均<0.05)。观察组护理总满意度高于对照组,差异有统计学意义(P<0.05)。结论针对临床危重患者,间歇充气加压泵的最佳使用时间为40 min,2次/d,可降低深静脉血栓,不增加组织损伤,且有助于改善股静脉血流动力学水平,得到较高的护理满意度。 Objective To explore the effect of different nursing time of intermittent pneumatic compression pump on deep vein thrombosis in clinical critical patients.Methods The clinical data of 160 critically ill patients admitted to the Department of Intensive Care Medicine of Juye County People's Hospital from June 2021 to June 2023 were retro⁃spectively selected.According to the different nursing time of intermittent pneumatic compression pump,they were di⁃vided into two groups,80 cases in each group.The use time of intermittent pneumatic compression pump in the con⁃trol group was 20 min,2 times/d,and that in the observation group was 40 min,2 times/d.The incidence of deep vein thrombosis,soft tissue injury,femoral vein hemodynamics and nursing satisfaction were compared between the two groups.Results The incidence of deep vein thrombosis in the observation group was 2.50%(2/80),which was lower than 11.25%(9/80)in the control group,and the difference was statistically significant(χ^(2)=4.783,P=0.029).The inci⁃dence rate of soft tissue injury was 2.50%(2/80)in the observation group and 1.25%(1/80)in the control group,there was no significant difference between the two groups(χ^(2)=0.000,P=1.000).After nursing,the blood flow peak velocity,average blood flow velocity and blood flow volume in the observation group were higher than those in the control group,and the differences were statistically significant(all P<0.05).The nursing satisfaction of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion For clinical critically ill patients,the optimal use time of intermittent pneumatic compression pump is 40 min,2 times/d,which can reduce deep vein thrombosis,does not increase tissue damage,and helps to improve the level of femoral vein hemodynamics,and get higher nursing satisfaction.
