基于湿性愈合疗法下重组牛碱性成纤维细胞生长因子外用凝胶联合凡士林纱布敷料用于拔甲术后创面换药的效果

Effect of Recombinant Bovine Basic Fibroblast Growth Factor External Gel Combined with Vaseline Gauze Dressing on Wound Dressing After Nail Removal Based on Wet Healing Therapy

目的探讨基于湿性疗法下重组牛碱性成纤维细胞生长因子外用凝胶联合凡士林敷料用于拔甲术后创面换药的效果。方法将2022年1月至2023年12月至厦门医学院附属第二医院进行换药的118例拔甲术后患者作为研究对象,运用随机法将其分为试验组58例,对照组60例。试验组运用重组牛碱性成纤维细胞生长因子外用凝胶联合凡士林纱布敷料进行换药,对照组采用传统凡士林敷料进行换药。就两组患者换药次数、愈合时间、并发症、疼痛数字评分(NRS)等进行比较。结果试验组患者换药次数少于对照组,愈合时间短于对照组(P<0.05);两组患者第一次换药发生粘连、出血、NRS评分差异无统计学意义(P>0.05)。第二次换药过程中试验组患者发生粘连例数、NRS评分均少于对照组(P<0.05)。第二次换药两组出血率相当,差异无统计学意义(P>0.05),实验组粘连、NRS评分低于对照组(P<0.05)。第三次换药试验组粘连、出血率、NRS评分优于对照组(P<0.05)。结论重组牛碱性成纤维细胞生长因子外用凝胶联合凡士林纱布敷料用于拔甲术后创面换药能加速创面愈合,减少并发症,减轻疼痛,临床疗效优于传统换药方法。

Objective To explore the effect of topical gel of recombinant bovine basic fibroblast growth factor combined with vaseline dressing on wound dressing after nail removal based on moist therapy.Methods From January 2022 to December 2023,A total of 118 patients who underwent dressing change in a Third-class hospital were taken as the research object,and randomly divided into experimental group(58 cases)and control group(60 cases).The experimental group used recombinant bovine basic fibroblast growth factor external gel combined with vaseline gauze dressing for dressing change,while the control group used traditional vaseline dressing for dressing change.The number of dressing changes,healing time,complications and pain numerical score(NRS)were compared between the two groups.Results The Times of dressing change in experimental group was less than that in control group,and the healing time was shorter than that in control group(P<0.05).There was no significant difference in adhesion,bleeding and NRS score between the two groups at the first dressing change(P>0.05).During the second dressing change,the number of adhesion cases and NRS score in experimental group were lower than those in control group(P<0.05).After the second dressing change,the bleeding rate of the two groups was similar,and the difference was not statistically significant(P>0.05).The adhesion and NRS scores of the experimental group were lower than those of the control group(P<0.05).The adhesion rate,bleeding rate and NRS score of experimental group were better than those of control group at the third dressing change(P<0.05).Conclusions Recombinant bovine basic fibroblast growth factor external gel combined with vaseline gauze dressing can accelerate wound healing,reduce complications and relieve pain after nail removal,and the clinical effect is obviously better than the traditional dressing change method.
