当归芍药散在眼底病的临证应用

Clinical Application of Danggui Shaoyao Powder in Eye Diseases

当归芍药散为出自《金匮要略》的活血利水的经方,从六经辨证分析,其适用于太阴、厥阴病证;从气、血、津液理论分析,其可理气治血利水;从五行理论分析,其可抑木扶土;从脏腑理论分析,其可柔肝健脾。临床上可用于气血同病、血水同病、肝脾不和、木乘土等症。气、血、津液三者互相影响,同源而互化,“血不利则为水”,当归芍药散为血水并治之方,针对气滞、血瘀、痰湿等病机,可以治疗糖尿病黄斑水肿、年龄相关性黄斑变性、视网膜静脉阻塞之血水同病的眼底疾病。针对这些致盲性眼病,探索新的治疗思路具有重要意义。通过分析中医理论基础,总结临床应用经验,为当归芍药散在现代眼科临床中的应用提供思路,并拓展其应用范围。

From the perspective of syndrome differentiation of the six meridians,the classical prescription Danggui Shaoyao Powder is applicable to Taiyin and Jueyin diseases.Analyzed through the theory of Qi,blood,and body fluids,it promotes harmony in Qi,regulates blood flow,and enhances diuresis.According to the theory of the five elements,it restrains the excess of wood and reinforces earth.Based on the theory of Zangfu organs,it nourishes liver and strengthens spleen.Clinically,it can be used for conditions of concurrent Qi and blood disorders,concurrent blood and water disorders,disharmony between liver and spleen,and the invasion of wood into earth.Qi,blood,and body fluids interact with each other,sharing the same origin and mutually transforming."When blood is not flowing smoothly,it turns into water".Danggui Shaoyao Powder,as a prescription for treating concurrent blood and water disorders,targets the pathogenesis of Qi stagnation,blood stasis,and phlegm-dampness,thereby treating ocular fundus diseases with concurrent blood and water disorders,such as diabetic macular edema,age-related macular degeneration,and retinal vein occlusion.In the face of these blinding eye diseases,it is of great significance to explore new treatment strategies.By analyzing the theoretical foundation of Traditional Chinese Medicine and summarizing clinical application experiences,this study aims to provide insights into the modern clinical application of Danggui Shaoyao Powder in ophthalmology and expand its scope of application.
