摘要
目的探讨半导体激光结合微波治疗老年带状疱疹(herpes zoster,HZ)的疗效。方法选择2022年8月—2024年2月上海市闵行区中心医院皮肤科就诊的120例老年HZ患者,按随机数字表法分为A组、B组和C组,每组40例。A组接受常规的药物治疗。B组接受除常规药物治疗之外,附加半导体激光治疗。C组接受常规药物治疗及半导体激光之外,附加微波治疗。结果C组(95.0%)的疗效高于A组(67.5%)和B组(82.5%),且组间相比较差异具有统计学意义(P<0.05)。治疗后,视觉模拟评分法(visual analog scale,VAS)评分显示C组(2.63±1.09)分显著低于A组(4.28±1.17)分和B组(3.79±1.03)分,且组间相比较差异有统计学意义(P<0.05)。并且C组的疼痛开始减轻时间(2.13±0.97)d及水疱开始结痂时间(3.46±0.78)d均早于A组(5.21±1.17)d、(6.12±0.07)d和B组(4.26±1.29)d、(4.95±1.01)d(P<0.05)。观察发现治疗开始后的第2周及第3周,C组的HZ后遗神经痛(postherpetic neuralgia,PHN)总发生率(16%、6%)显著低于A组(31%、23%)和B组(22%、14%)(P<0.05)。结论半导体激光联合微波治疗老年HZ的疗效显著。
Objective To explore the therapeutic effects of semiconductor laser combined with microwave on herpes zoster(HZ)in the elderly.Methods 120 elderly HZ patients treated in Department of Dermatology,Central Hospital of Minhang District from August 2022 to February 2024 were selected and divided into group A,group B and group C according to random number table method,with 40 cases in each group.The group A received conventional drug therapy.The group B received semiconductor laser therapy in addition to conventional drug therapy.The group C received microwave therapy in addition to conventional drug and semiconductor laser therapy.Results The therapeutic effect of the group C(95.0%)was higher than those of group A(67.5%)and group B(82.5%),and the difference among the 3 groups was statistically significant(P<0.05).After treatment,the Visual Analog Scale(VAS)score of the group C(2.63±1.09)was significantly lower than those of group A(4.28±1.17)and group B(3.79±1.03),and the difference among the 3 groups was statistically significant(P<0.05).The pain relief time(2.13±0.97d)and blister scabbing formation time(3.46±0.78 d)of the group C were earlier than those of the group A(5.21±1.17 d and 6.12±0.07 d)and group B(4.26±1.29 d and 4.95±1.01 d)(P<0.05).Within 2 and 3 weeks after treatment,the overall incidence of postherpetic neuralgia(PHN)in the group C was significantly lower(16%,6%)than those in the group A(31%,23%)and group B(22%,14%)(P<0.05).Conclusion The combination of semiconductor laser and microwave therapy has significant therapeutic effects on HZ in elderly patients.
