特发性矮小症患儿血清25-羟维生素D、胰岛素样生长因子-1水平与体格发育指标的相关性分析

摘要 目的:分析特发性矮小症患儿血清25-羟维生素D[25(OH)D]及胰岛素样生长因子-1(IGF-1)水平与体格发育指标的相关性。方法:选取2021年6月—2023年6月于云浮市妇幼保健院儿童保健门诊确诊为特发性矮小症的患儿146例为观察组,并选取同时期于云浮市妇幼保健院体检的健康儿童86例为对照组。比较两组25(OH)D、IGF-1水平及体质量指数(BMI)、身高,并分析25(OH)D、IGF-1水平与身高、BMI的相关性。结果:观察组血清25(OH)D、IGF-1水平、身高、BMI低于对照组(P<0.001)。Spearman相关性分析显示,血清25(OH)D水平与身高呈正相关(r=0.334,P<0.001),血清25(OH)D水平与BMI呈正相关(r=0.620,P<0.001);血清IGF-1水平与身高呈正相关(r=0.729,P<0.001),血清IGF-1水平与BMI呈正相关(r=0.387,P<0.001)。结论:特发性矮小症患儿的血清25(OH)D、IGF-1水平及身高、BMI低于健康儿童,且血清25(OH)D、IGF-1水平与身高、BMI呈正相关,需定期筛查相关指标,实现疾病早期干预。 Objective:To analyze the correlation between serum 25-hydroxyvitamin D[25(OH)D]and insulin-like growth factor-1(IGF-1)levels and physical development indicators in children with idiopathic dwarfism.Methods:One hundred and forty-six children diagnosed with idiopathic dwarfism at the Child Health Clinic of Yunfu Maternal and Child Health Hospital from June 2021 to June 2023 were selected as the observation group.And eighty-six healthy children who underwent physical examination in Yunfu Maternal and Child Health Hospital during the same period were selected as the control group.We compared the levels of 25(OH)D and IGF-1,body mass index(BMI)and height between the two groups,and analyzed the correlation between 25(OH)D and IGF-1 levels and height and BMI.Results:The serum 25(OH)D,IGF-1 levels,height,and BMI of the observation group were lower than those of the control group(P<0.001).Spearman correlation analysis showed that serum 25(OH)D level was positively correlated with height(r=0.334,P<0.001),serum 25(OH)D levels were positively correlated with BMI(r=0.620,P<0.001);serum IGF-1 levels were positively correlated with height(r=0.729,P<0.001)and serum IGF-1 levels were positively correlated with BMI(r=0.387,P<0.001).Conclusion:The serum 25(OH)D and IGF-1 levels and height and BMI of children with idiopathic dwarfism were lower than those of healthy children,and the serum 25(OH)D and IGF-1 levels were positively correlated with height and BMI,so it is necessary to screen the related indexes regularly to realize early intervention of the disease.
