急诊猝死患者家属哀伤辅导方案的构建

Construction of a grief counseling program for family members of patients with sudden death in the emergency department

目的 :构建急诊猝死患者家属哀伤辅导方案,为缓解猝死患者家属的哀伤情绪、降低猝死患者家属延长哀伤障碍发生风险提供参考。方法 :在文献研究的基础上结合临床实践经验初步拟订急诊猝死患者家属哀伤辅导方案,通过德尔菲专家函询法进一步修订方案。结果 :2轮德尔菲函询专家权威系数为0.904;第2轮专家函询后,各个条目的重要性和可操作性的变异系数均为0.05~0.24,肯德尔和谐系数分别为0.174、0.215(P<0.001)。最终方案包括组建哀伤辅导团队、哀伤辅导需求评估、哀伤辅导实施、哀伤辅导随访4个一级条目、9个二级条目、28个三级条目。结论 :构建的急诊猝死患者家属哀伤辅导方案具有科学性、实用性和可靠性,可为急诊实施猝死患者家属哀伤辅导工作提供参考。

Objective:To construct a grief counseling program for family members of patients with sudden death in the emergency department,and to alleviate their grief and reduce the risk of prolonged grief disorder.Methods:The grief counseling program was developed based on literature review and clinical experience.The program was further revised and its feasibility was demonstrated through the Delphi method.Results:The authority coefficient of the Delphi expert consultations was 0.904.After the second round of expert consultations,the coefficients of variation for the importance and feasibility of each item ranged from 0.05 to 0.24,respectively.The Kendall concordance coefficients were 0.174 and 0.215(P<0.001).The final program comprised 4 primary items,9 secondary items,and 28 tertiary items,including the establishment of a grief counseling team,assessment of grief counseling needs,implementation of grief counseling,and follow-up.Conclusion:The grief counseling program for family members of patients with sudden death in the emergency department is scientific,practical,and reliable,providing a reference for implementing grief counseling for family members of sudden death patients in clinical settings.
