糖尿病足区域协同治疗平台的构建及应用

Construction and application of regional collaborative treatment platform for diabetic foot

目的:研发糖尿病足区域协同治疗平台,并分析其应用效果,为糖尿病足延续护理提供参考。方法 :选择1家三级医院及医联体内的12家社区卫生服务中心使用该平台,依托三级医院专家开展线上指导、线下会诊、转诊、宣教等服务。比较平台使用前后糖尿病足治疗愈合率、好转率,并统计平台使用情况和护士对平台的满意度。结果 :干预4周时实验组1级和2级糖尿病足治疗愈合率高于对照组(P<0.05),干预6周时实验组2级糖尿病足治疗愈合率高于对照组(P<0.05),干预12周时实验组4级糖尿病足治疗好转率高于对照组(P<0.05),三级医院各科室联络护士对平台的满意度为96.6%,社区护士对平台的满意度为100.0%。结论 :糖尿病足区域协同治疗平台的研发和应用,促进了糖尿病足患者创面的愈合,提高了一线护士的满意度,有助于开展延续护理。

Objective:To develop a regional collaborative treatment platform for diabetic foot and explore its application effect,and to provide a reference for the continuing care.Methods:The platform was used in one tertiary hospital and twelve community health service centers within the same medical alliance network,and online technical support,offline consultation,referral,education,and other services were provided by experts from the tertiary hospital.The cure rate and improvement rate of diabetic foot were compare before and after using the platform,and the usage of the platform and the satisfaction of nurses were analyzed.Results:The healing rate of grade 1 and grade 2 diabetic foot in the experimental group was higher than that in the control group at 4 weeks(P<0.05),the healing rate of grade 2 diabetic foot in the experimental group was higher than that in the control group at 6 weeks(P<0.05),and the improvement rate of grade 4 diabetic foot was higher at 12 weeks after the usage of the platform in the experimental group than those in the control group(P<0.05).The satisfaction with the platform of the liaison nurses in the tertiary hospital was 96.6%,and the satisfaction of community nurses was 100.0%.Conclusion:The development and application of the regional cooperative treatment platform for diabetic foot can promote the wound healing of diabetic foot,improve the satisfaction of nurses,and contributed to the continuous nursing care.
