中医导引术联合中西医结合疗法对2型糖尿病足溃疡康复的影响

Effect of Daoyin therapy of traditional Chinese medicine combined with integrated traditional Chinese and western medicine in the rehabilitation of type 2 diabetic foot ulcers

目的评估中医导引术联合中西医结合治疗对2型糖尿病足溃疡康复的疗效。方法回顾性分析2017年1月至2021年4月在首都医科大学附属北京中医医院接受治疗的71例2型糖尿病足溃疡患者资料。患者按治疗方法分导引组(36例)和对照组(35例),其中导引组接受导引术结合中西医结合疗法,对照组仅接受中西医结合疗法,比较两组患者的疗效和相关临床指标。结果对照组治疗后心功能分级、降钙素原(procalcitonin,PCT)和C反应蛋白(C-reactive protein,CRP)指标和治疗前的差异有统计学意义(P<0.05),而导引组的踝肱指数(ankle brachial index,ABI)、经皮氧分压(transcutanous oxygen pressure,TcPO2)、疼痛评分、跛行距离、心功能分级、PCT和CRP等指标和治疗前比较差异有统计学意义(P<0.05)。两组患者治疗后ABI、TcPO2、疼痛评分、跛行距离、心功能分级和CRP指标的差异有统计学意义(P<0.05)。导引组患者出现不良事件25次,发生率1.15%,无跌倒损伤、骨折及膝、踝关节损伤等情况出现。结论导引术结合中西医结合疗法对2型糖尿病足溃疡康复具有积极的影响,在促进创面愈合、改善疼痛、缓解炎症、改善心脏功能方面显出益处。

Objective To evaluate the efficacy of Daoyin therapy of traditional Chinese medicine combined with integrated traditional Chinese and western medicine in the rehabilitation of type 2 diabetic foot ulcers.Methods The data of 71 patients with type 2 diabetic foot ulcers who were treated at Beijing Hospital of Traditional Chinese Medicine,Capital Medical University from January 2017 to April 2021 were analyzed retrospectively.The patients were divided into Daoyin group(36 cases)and control group(35 cases)according to treatment methods.The Daoyin group received Daoyin surgery combined with integrated traditional Chinese and western medical treatments,while the control group only received integrated traditional Chinese and western medical treatments.The efficacy and related clinical indicators of the two groups were compared.Results In the control group,the differences in cardiac function classification,procalcitonin(PCT)and C-reactive protein(CRP)after treatment were statistically significant than those before treatment(P<0.05).In the Daoyin group,there were statistically significant differences in ankle brachial index(ABI),transcutaneous oxygen pressure(TcPO2),pain score,claudication distance,cardiac function classification,PCT and CRP compared with those before treatment(P<0.05).There were statistically significant differences in ABI,TcPO2,pain score,claudication distance,cardiac function classification and CRP indicators between the two groups after treatment(P<0.05).Patients in the Daoyin group experienced 25 adverse events,with an incidence rate of 1.15%.There were no falls,fractures,knee or ankle injuries occured.Conclusion Daoyin therapy combined with integrated traditional Chinese and western medical treatments has a positive impact on the rehabilitation of type 2 diabetic foot ulcers,showing benefits in promoting wound healing,improving pain,relieving inflammation and improving heart function.
