输尿管镜下钬激光碎石取石术治疗老年泌尿系统结石的疗效及安全性分析

Efficacy and Safety of Ureteroscopic Holmium Laser Lithotripsy in the Treatment of Urinary Calculi in the Elderly

目的对比气压弹道碎石和输尿管镜下钬激光碎石两种术式治疗老年泌尿系统结石的临床治疗效果。方法随机选取2021年2月—2024年2月惠安新惠兴老年病医院收治的100例老年泌尿系统结石患者为研究对象,根据不同手术方式分为对照组(n=50)和观察组(n=50)。对照组接受传统气压弹道碎石术治疗,观察组接受输尿管镜下钬激光碎石取石术治疗。对比两组的手术基本情况、一期碎石成功率、碎石疗效以及并发症发生率。结果观察组手术时间、术后下床活动时间、术后住院时间均短于对照组,差异有统计学意义(P均<0.05)。观察组一期碎石成功率为100.00%(50/50),高于对照组的86.00%(43/50),差异有统计学意义(χ^(2)=5.530,P<0.05)。观察组治疗总有效率高于对照组,并发症总发生率低于对照组,差异有统计学意义(P均<0.05)。结论输尿管镜下钬激光碎石取石术治疗老年泌尿系统结石在治疗效果及安全性方面更具优势。

Objective The clinical effects of pneumatic lithotripsy and ureteroscopic holmium laser lithotripsy in the treatment of urinary calculi in the elderly were compared.Methods A total of 100 elderly patients with urinary calculi admitted to Hui'an Xinhuixing Geriatric Hospital from February 2021 to February 2024 were randomly selected as the research objects.According to different surgical methods,they were divided into control group(n=50)and observation group(n=50).The control group was treated with traditional pneumatic lithotripsy,and the observation group was treated with ureteroscopic holmium laser lithotripsy.The basic operation conditions,the succes rate of first stage lithptripsy,lithotripsy efficacy and complication rate of the two groups were compared.Results The operation time,postoperative ambulation time and postoperative hospital stay in the observation group were shorter than those in the control group,and the differences were statistically significant(all P<0.05).The success rate of one-stage lithotripsy in the observation group was 100.00%(50/50),which was higher than 86.00%(43/50)in the control group,and the difference was statistically significant(χ^(2)=5.530,P<0.05).The total effective rate of treatment in the observation group was higher than that in the control group,and the total incidence of complications was lower than that in the control group,the differences were statistically significant(both P<0.05).Conclusion Holmium laser lithotripsy under ureteroscopy has more advantages in elderly urinary system stones treatment in terms of therapeutic effect and safety.
