专科护理门诊实施责任制整体护理对留置PICC导管相关性皮肤损伤患者的影响

Effect of Responsibility System Holistic Nursing on Patients with Indwelling PICC Catheter-related Skin Injury in Specialist Nursing Clinic

目的分析专科护理门诊实施责任制整体护理对留置外周静脉植入的中心静脉导管(peripherally inserted central catheter,PICC)相关性皮肤损伤患者的影响。方法便利选取2023年1月—2024年5月广州市南沙区人民医院收治的84例留置PICC导管相关性皮肤损伤患者为研究对象,根据不同的护理方法分为两组,各42例。对照组实施常规护理,研究组实施责任制整体护理。比较两组皮肤损伤程度、皮肤瘙痒程度、皮肤损伤的愈合时间、不良反应发生率、负性情绪和护理满意度。结果研究组急性皮肤损伤量表评分、瘙痒症状评估量表评分均低于对照组,皮肤损伤的愈合时间短于对照组,差异有统计学意义(P均<0.05)。研究组不良反应发生率为4.76%(2/42),低于对照组的19.05%(8/42),差异有统计学意义(χ^(2)=4.086,P<0.05)。研究组焦虑自评量表评分和抑郁自评量表评分均低于对照组,差异有统计学意义(P均<0.05)。研究组护理满意度高于对照组,差异有统计学意义(P<0.05)。结论责任制整体护理应用于留置PICC导管相关性皮肤损伤患者中,能够改善患者的皮肤损伤程度和瘙痒程度,进一步缩短愈合时间,有效减轻患者的负性情绪,减少不良反应的发生,提高患者的护理满意度。

Objective To analyze the effect of responsibility system holistic nursing on patients with indwelling peripherally inserted central catheter(PICC)catheter-related skin injury in specialist nursing clinic.Methods A total of 84 patients with indwelling PICC catheter-related skin injury admitted to Guangzhou Nansha District People's Hospital from January 2023 to May 2024 were conveniently selected as the research objects.According to different nursing methods,they were divided into two groups,with 42 cases in each group.The control group were given routine nursing,and the study group were given responsibility system holistic nursing.The degree of skin injury,the degree of skin itching,the healing time of skin injury,the incidence of adverse reactions,negative emotions and nursing satisfaction were compared between the two groups.Results The Acute Skin Injury Scale score and Itch Severity Scale score of the study group were lower than those of the control group,and the healing time of skin injury was shorter than that of the control group,and the differences were statistically significant(all P<0.05).The incidence of adverse reactions in the study group was 4.76%(2/42),which was lower than 19.05%(8/42)in the control group,the difference was statistically significant(χ^(2)=4.086,P<0.05).The scores of the Self-rating Depression Scale and Self-rating Depression Scale of the study group were lower than those of the control group,and the differences were statistically significant(both P<0.05).The nursing satisfaction of the study group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion The application of responsibility system holistic nursing in patients with skin injury associated with PICC catheters can improve the degree of skin injury and itching of patients,further shorten the healing time,effectively reduce the negative emotions of patients,reduce the occurrence of adverse reactions,and improve the nursing satisfaction of patients.
