基于艾儒棣教授守中祛邪思想探讨急性放射性皮炎的分期治疗

Staged Treatment of Acute Radiation Dermatitis Based on Professor Ai Rudi′s Thought of Guarding Middle and Dispelling Evil

急性放射性皮炎(acute radiation dermatitis,ARD)是肿瘤放射过程中的常见并发症,目前尚无明确治疗指南,常规处理ARD存在难愈合,易复发且不良反应较多等问题。本文以艾儒棣教授守中祛邪思想为治则,分析ARD的病因病机、分期辨证,针对ARD火毒外侵,阴血内耗的病理特点,确立外祛火毒,内守阴血的治法,并提出五皮饮+紫草油治疗火毒伏经证;凉血消风散+紫藤油治疗火毒燔灼证;补血解毒汤+七星丹/海浮散/玉红膏治疗火毒焮腾证的中医辨证论治思想及用药规律,以期为临床治疗急性放射性皮炎提供新思路。

Acute radiation dermatitis(ARD)is a common complication during tumor radiation,and there is no clear treatment guideline,and conventional management of ARD is difficult to heal,prone to recurrence and with high side effects.Based on Professor Ai Rudi′s idea of keeping the Middle ground and dispelling Evil as the rule of treatment,we analyzed the etiology and pathogenesis of ARD,staged and identified the symptoms,established the treatment method of dispelling Fire Poison externally and guarding the Yin and Blood internally in response to the pathological characteristics of ARD,and proposed the TCM theory of syndrome differentiation and medication law:Wupi Drink+Comfrey Oil for the treatment of Fire-Toxin lies in the Meridians;Cooling Blood and Eliminating Wind Powder+Wisteria Oil for treatment of Fire-Toxin Burnt Symptoms;Buxue Jiedu Decoction+Qixin Elixir/Haifu Powder/Yuhong Paste for the treatment of Fire-Toxin inflammation vacuolated evidence with the aim of providing a new thought for the clinical treatment of acute radio dermatitis.
