保留膀胱颈经尿道等离子体前列腺剜除术治疗前列腺增生的临床效果研究

Clinical Effect of Transurethral Enucleative Resection of Prostate with Bladder Neck Preservation in the Treatment of Benign Prostatic Hyperplasia

目的探究前列腺增生(benign prostatic hyperplasia,BPH)患者应用保留膀胱颈经尿道等离子体前列腺剜除术(transurethral enucleative resection of prostate,TUERP)的效果。方法随机选取2022年1月—2023年12月盐城市第一人民医院收治的326例BPH患者为研究对象。根据手术方法不同分为研究组、对照组,各163例。参照组行经尿道等离子电切术(transurethral resection prostate,TURP)治疗,研究组行保留膀胱颈TUERP治疗。比较两组的手术指标、排尿功能、性功能。结果研究组术中出血量高于对照组,差异有统计学意义(P<0.05)。术后,研究组国际前列腺症状评分为(7.26±1.33)分,最大尿流率为(20.98±3.65)mL,残余尿为(15.02±3.33)mL,最大逼尿肌压力为(63.25±6.41)cmH_(2)O,优于对照组的(8.79±1.51)分、(18.02±3.31)mL、(18.97±3.71)mL、(72.59±6.36)cmH_(2)O,差异有统计学意义(t=9.708、7.670、10.116、13.206,P均<0.05)。研究组国际勃起功能指数-5和射精功能评分均高于对照组,差异有统计学意义(P均<0.05)。结论保留膀胱颈TUERP能够改善BPH患者的排尿功能及性功能,减少患者的手术损伤。

Objective To investigate the effect of transurethral enucleative resection of prostate(TUERP)with bladder neck preservation in patients with benign prostatic hyperplasia(BPH).Methods A total of 326 BPH patients admitted to Yancheng NO.1 People's Hospital from January 2022 to December 2023 were randomly selected as the research objects.According to different surgical methods,they were divided into study group and control group,with 163 cases in each group.The control group was treated with transurethral resection prostate(TURP),and the study group was treated with bladder neck TUERP.The surgical indicators,urinary function and sexual function were compared be⁃tween the two groups.Results The intraoperative blood loss in the study group was higher than that in the control group,and the difference was statistically significant(P<0.05).After operation,the International Prostate Symptom Score of the study group was(7.26±1.33)points,the maximum flow rate rate was(20.98±3.65)mL,the postvoid re⁃sidual was(15.02±3.33)mL,and the maximum detrusor pressure was(63.25±6.41)cmH_(2)O,which were better than those of the control group(8.79±1.51)points,(18.02±3.31)mL,(18.97±3.71)mL,(72.59±6.36)cmH_(2)O,the differ⁃ences were statistically significant(t=9.708,7.670,10.116,13.206,all P<0.05).The International Index of Erectile Function-5 and Chinese Index of Sexual Funetion for Premature Ejaculation score of the study group were higher than those of the control group,and the differences were statistically significant(both P<0.05).Conclusion The TUERP with bladder neck can improve the urination function and sexual function of BPH patients and reduce the surgical in⁃jury of patients.
