输尿管软镜钬激光碎石术在输尿管上段结石患者中的应用效果研究

Study on the Application Effect of Flexible Ureteroscopic Holmium Laser Lithotripsy in Patients with Upper Ureteral Calculi

目的分析输尿管软镜钬激光碎石术在输尿管上段结石患者中的应用效果。方法简单随机选取2023年1月—2024年4月昆山市第四人民医院收治的60例输尿管上段结石患者为研究对象,根据不同治疗方法分为观察组(30例,输尿管软镜钬激光碎石术治疗)与对照组(30例,输尿管硬镜钬激光碎石术)。比较两组的住院时间,疼痛评分、血尿时间、炎症因子水平、临床疗效、并发症发生情况。结果观察组住院时间为(5.15±1.43)d,血尿时间为(2.17±0.45)d,均短于对照组的(7.56±1.32)d、(4.12±1.01)d,疼痛评分为(2.09±0.54)分,低于对照组的(3.83±1.02)分,差异有统计学意义(t=6.783、9.659、8.258,P均<0.05)。治疗后,观察组降钙素原、C反应蛋白、白细胞水平低于对照组,差异有统计学意义(P均<0.05)。观察组治疗显效率高于对照组,差异有统计学意义(P<0.05)。治疗后,观察组并发症发生率低于对照组,但差异无统计学意义(P>0.05)。结论输尿管软镜钬激光碎石术的疗效确切,可以提高治疗有效率,改善炎症因子,安全性较高。

Objective To analyze the application effect of flexible ureteroscopic holmium laser lithotripsy in patients with upper ureteral calculi.Methods From January 2023 to April 2024,60 patients with upper ureteral calculi ad⁃mitted to Kunshan Fourth People's Hospital were simple randomly selected as the research objects.According to different treatment methods,they were divided into observation group(30 cases,flexible ureteroscopic holmium la⁃ser lithotripsy)and control group(30 cases,rigid ureteroscopic holmium laser lithotripsy).The hospitalization time,pain score,hematuria time,inflammatory factor level,treatment effect and complications were compared between the two groups.Results The hospitalization time of the observation group was(5.15±1.43)d,and the hematuria time was(2.17±0.45)d,which were shorter than those of the control group(7.56±1.32)d and(4.12±1.01)d.The pain score was(2.09±0.54)points,which was lower than that of the control group(3.83±1.02)points,and the differences were statistically significant(t=6.783,9.659,8.258,all P<0.05).After treatment,the levels of procalcitonin,Creactive protein and white blood cells in the observation group were lower than those in the control group,and the dif⁃ferences were statistically significant(all P<0.05).The effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the incidence of complica⁃tions in the observation group was lower than that in the control group,but the difference was not statistically signifi⁃cant(P>0.05).Conclusion The curative effect of flexible ureteroscopic holmium laser lithotripsy is definite,which can improve the effective rate of treatment,improve inflammatory factors and high safety.
