基于“SMART”康复目标的协同护理模式对老年冠心病患者心脏康复的影响

The Effect of Collaborative Care Model Based on"Smart"Rehabilitation Goal on Cardiac Rehabilitation in Elderly Patients with Coronary Heart Disease

目的 分析基于“SMART”康复目标的协同护理模式(collaborative care model, CCM)对老年冠状动脉粥样硬化性心脏病(coronary heart disease, CHD)患者心脏康复的影响。方法 方面选取2022年12月—2023年9月南通瑞慈医院心血管内科收治的84例老年CHD患者为研究对象,根据护理方法不同分为观察组与对照组,各42例。对照组给予常规康复措施,观察组给予基于“SMART”康复目标的协同护理措施,均干预6个月,比较两组的6 min步行试验距离、自我效能及生活质量。结果 干预前,两组的6 min步行试验距离、自我效能评分、生活质量评分比较,差异无统计学意义(P均>0.05)。干预6个月后,观察组的6 min步行试验距离为(318.76±15.60)m,长于对照组的(278.98±14.17)m,差异有统计学意义(t=12.233,P<0.05)。干预6个月后,两组的自我效能评分和生活质量评分均优于干预前,且观察组优于对照组,差异有统计学意义(P均<0.05)。结论 基于“SMART”康复目标的协同护理模式在老年CHD患者心脏康复的应用能促进患者康复,改善患者自我效能感,提高生活质量。

Objective To analyze the effect of collaborative care model(CCM)based on"SMART"rehabilitation goals on cardiac rehabilitation in elderly patients with coronary heart disease(CHD).Methods A total of 84 elderly CHD pa⁃tients admitted to the Department of Cardiology,Nantong Ruici Hospital from December 2022 to September 2023 were conveniently selected as the research objects.According to different nursing methods,they were divided into observa⁃tion group and control group,with 42 cases in each group.The control group was given routine rehabilitation mea⁃sures,and the observation group was given collaborative nursing measures based on"SMART"rehabilitation goals.Both groups were intervened for 6 months.The 6-minute walking test distance,self-efficacy and quality of life were compared between the two groups.Results Before intervention,there were no significant differences in 6-minute walk⁃ing test distance,self-efficacy score and quality of life score between the two groups(all P>0.05).After 6 months of in⁃tervention,the 6-minute walking test distance of the observation group was(318.76±15.60)m,which was longer than that of the control group(278.98±14.17)m,and the difference was statistically significant(t=12.233,P<0.05).After 6 months of intervention,the self-efficacy scores and quality of life scores of the two groups were better than those be⁃fore intervention,and the observation group was better than the control group,the differences were statistically signifi⁃cant(all P<0.05).Conclusion The application of collaborative nursing model based on"SMART"rehabilitation goal in cardiac rehabilitation of elderly patients with CHD can promote patients'rehabilitation,improve patients'self efficacy and improve their quality of life.
