超声引导下股神经阻滞在髌骨骨折患者术后镇痛中的应用效果

Effect of Ultrasound-guided Femoral Nerve Block on Postoperative Analgesia on Patients with Patellar Fracture

目的分析在髌骨骨折术后镇痛中选用超声引导下股神经阻滞的效果。方法回顾性选取昆山市康复医院于2021年7月—2024年1月接收的40例髌骨骨折患者的临床资料,根据镇痛方式的不同分为对照组(20例)、观察组(20例)。对照组采取术后静脉自控镇痛,观察组采取超声引导股神经阻滞镇痛。比较两组镇痛药物使用剂量、疼痛程度评分、不良反应(恶心呕吐、心律不齐、头晕头痛、低血压)发生率。结果观察组术后使用舒芬太尼总剂量为(81.17±2.96)mL,低于对照组的(84.25±3.56)mL;镇痛泵按压次数为(17.26±3.82)次,少于对照组的(20.06±4.15)次,差异有统计学意义(t=2.975、2.220,P均<0.05)。观察组不良反应总发生率为0,低于对照组的20.00%(4/20),差异有统计学意义(P<0.05)。结论基于超声引导的股神经阻滞对髌骨骨折手术患者具有良好的效果,可以缓减疼痛,安全性较高。

Objective To analyze the effect of ultrasound-guided femoral nerve block in postoperative analgesia of patellar fracture.Methods The clinical data of 40 patients with patellar fracture received in Kunshan Rehabilitation Hospital from July 2021 to January 2024 was retrospectively selected and divided into control group(20 cases)and observation group(20 cases)according to different analgesic methods.The control group received postoperative patient-controlled intravenous analgesia,and the observation group received ultrasoundguided femoral nerve block analgesia.The dosage of analgesic drugs,pain score and incidence of adverse reactions(nausea and vomiting,arrhythmia,dizziness and headache,hypotension)were compared between the two groups.Results The total dose of sufentanil in the observation group was(81.17±2.96)mL,which was lower than(84.25±3.56)mL in the control group,and the number of analgesic pump compressions in the observation grouo was(17.26±3.82)times,which was less than(20.06±4.15)times in the control group,the differences were statistically significant(t=2.975,2.220,both P<0.05).The total incidence of adverse reactions in the observation group was 0,which was lower than 20.00%(4/20)in the control group,and the difference was statistically significant(P<0.05).Conclusion For patients with patella fracture surgery,the femoral nerve block based on ultrasound guidance has good results,with certain advantages for pain relief,high safety,and certain research significance.
