经阴道彩色多普勒超声诊断异位妊娠的临床应用效果

Clinical Application Effect of Transvaginal Color Doppler Ultrasound in the Diagnosis of Ectopic Pregnancy

目的探讨异位妊娠患者采用经阴道彩色多普勒超声诊断的价值及效果。方法选取2021年1月—2023年1月金昌市中心医院影像科收治的80例疑似异位妊娠患者为研究对象,入组患者均行腹部超声与阴道超声检查,以手术结果为金标准,比较不同检查方法的检出率,并计算诊断效能。结果经手术结果确诊58例,经腹部超声检出阳性44例,经阴道超声检出阳性51例,两种超声联合检出阳性55例。经腹部超声联合经阴道超声诊断异位妊娠的准确度为88.75%(71/80)、灵敏度为89.66%(52/58)、特异度为86.36%(19/22)、阳性预测值为94.55%(52/55)、阴性预测值为76.00%(19/25),高于经腹部超声检查[62.50%(50/80)、62.07%(36/58)、63.64%(14/22)、81.82%(36/44)、38.89%(14/36)]及经阴道超声检查[73.75%(59/80)、75.86%(44/58)、68.18%(15/22)、86.27%(44/51)、51.72%(15/29)],差异有统计学意义(χ^(2)=6.392、7.361、6.093、5.582、6.141,P均<0.05)。结论经阴道彩色多普勒超声和经腹部超声用于异位妊娠诊断的阳性检出率较高,且二者联合检查能提高诊断效能。

Objective To investigate the value and effect of transvaginal color Doppler ultrasound in the diagnosis of ectopic pregnancy.Methods A total of 80 patients with suspected ectopic pregnancy admitted to the Department of Imaging of Jinchang Central Hospital from January 2021 to January 2023 were selected as the research objects.The enrolled patients were examined by abdominal ultrasound and vaginal ultrasound.The surgical results were used as the gold standard.The detection rates of different examination methods was compared,and the diagnostic efficacy was calculated.Results 58 cases were diagnosed by surgical results,44 cases were positive by trans abdominal ultrasound,51 cases were positive by transvaginal ultrasound,and 55 cases were positive by combined ultrasound.The accuracy,sensitivity,specificity,positive predictive value and negative predictive value of transabdominal ultrasound combined with transvaginal ultrasound in the diagnosis of ectopic pregnancy were 88.75%(71/80),89.66%(52/58),86.36%(19/22),94.55%(52/55)and 76.00%(19/25),respectively,which were higher than those of transabdominal ultrasound examination[62.50%(50/80),62.07%(36/58),63.64%(14/22),81.82%(36/44),38.89%(14/36)]and transvaginal ultrasound[73.75%(59/80),75.86%(44/58),68.18%(15/22),86.27%(44/51),51.72%(15/29)],and the differences were statistically significant(χ^(2)=6.392,7.361,6.093,5.582,6.141,all P<0.05).Conclusion The positive detection rate of transvaginal color Doppler ultrasound and trans abdominal ultrasound in the diagnosis of ectopic pregnancy is high,and the combined examination of the two can improve the diagnostic efficiency.
