心脏超声联合颈部血管超声诊断老年冠心病的应用价值

Application Value of Cardiac Ultrasound Combined with Cervical Vascular Ultrasound in the Diagnosis of Elderly Coronary Heart Disease

目的探讨心脏超声联合颈部血管超声诊断老年冠状动脉粥样硬化性心脏病(简称冠心病)的价值。方法选择2022年1月—2023年12月丹阳市人民医院收治的72例疑似老年冠心病患者为研究对象,均予心脏超声和颈部血管超声检查,以冠状动脉造影为金标准,评估超声联合检查诊断效能,并分析联合检查对评估动脉狭窄的诊断符合率。结果联合检查的准确度为97.22%(70/72)、灵敏度为98.28%(57/58),高于心脏超声的86.11%(62/72)、84.48%(49/58)与颈部血管超声的84.72%(61/72)、81.03%(47/58),差异有统计学意义(χ^(2)=7.104、9.098,P均<0.05)。联合检测对动脉轻度狭窄诊断符合率为96.43%(27/28),高于心脏、颈部血管超声的71.43%(20/28)、67.86%(19/28),差异有统计学意义(χ^(2)=8.061,P<0.05)。结论心脏超声联合颈部血管超声对老年冠心病的诊断效能高,可有效识别动脉狭窄程度。

Objective To investigate the value of cardiac ultrasound combined with cervical vascular ultrasound in the diagnosis of coronary heart disease in the elderly.Methods A total of 72 elderly patients with suspected coronary heart disease treated in Danyang People's Hospital from January 2022 to December 2023 were selected as the research objects.All patients were examined by cardiac ultrasound and cervical vascular ultrasound.Coronary angiography was used as the gold standard to evaluate the diagnostic efficacy of ultrasound combined examination,and analyze the diagnostic coincidence rate of combined examination in evaluating arterial stenosis.Results The accuracy and sensitivity of combined examination were 97.22%(70/72)and 98.28%(57/58),which were higher than 86.11%(62/72),84.48%(49/58)of cardiac ultrasound and 84.72%(61/72),81.03%(47/58)of cervical vascular ultrasound,and the differences were statistically significant(χ^(2)=7.104,9.098,both P<0.05).The diagnostic coincidence rate of combined examination for mild arterial stenosis was 96.43%(27/28),which was higher than 71.43%(20/28)and 67.86%(19/28)of cardiac and cervical vascular ultrasound,and the difference was statistically significant(χ^(2)=8.061,P<0.05).Conclusion Cardiac ultrasound combined with cervical vascular ultrasound has high diagnostic efficiency for elderly coronary heart disease,which can effectively the identify degree of arterial stenosis.
