磁共振动态增强成像及扩散加权成像联合检查对乳腺癌的诊断价值分析

Diagnostic Value of Dynamic Enhanced Magnetic Resonance Imaging and Diffusion Weighted Imaging in Breast Cancer

目的分析磁共振动态增强成像(dynamic contrast-enhanced magnetic resonance imaging,DCE-MRI)、扩散加权成像(diffusion weighted imaging,DWI)联合诊断乳腺癌的效能。方法选取2022年6月—2023年6月济南市中西医结合医院收治的100例乳腺占位性病变患者为研究对象,均开展DCE-MRI、DWI检查,以病理切片检查作为金标准,分析DCE-MRI、DWI单独及联合检查的诊断效能。结果100例乳腺占位性病变患者经病理检查结果显示,乳腺癌55例,乳腺良性病变45例;经DCE-MRI检出乳腺癌50例,经DWI检出乳腺癌49例,DCE-MRI联合DWI检出乳腺癌54例。联合检查的灵敏度、准确度均高于DCE-MRI、DWI单独诊断,差异有统计学意义(P均<0.05)。联合检查与金标准一致性kappa值为0.940。绘制受试者操作特征曲线显示,DCE-MRI、DWI单独及联合诊断乳腺癌的曲线下面积均>0.70,尤以DCE-MRI、DWI联合诊断价值最高。结论DCE-MRI、DWI联合应用,可显著提高乳腺癌的诊断效能,与病理切片检查结果一致性较好。

Objective To analyze the efficacy of dynamic enhanced magnetic resonance imaging(DCEMRI)and diffusion weighted imaging(DWI)in the diagnosis of breast cancer.Methods A total of 100 patients with breast space-occupying lesions treated in Jinan Hospital of Integrated Traditional Chinese and Western Medicine from June 2022 to June 2023 were selected as the study objests.DCE-MRI and DWI were performed,and pathological section examination was taken as the gold standard.The diagnostic efficacy of DCE-MRI and DWI alone and combined were analyzed.Results The pathological examination results of 100 patients with breast space occupying lesions showed that 55 cases of breast cancer and 45 cases of benign breast lesions.Breast cancer was detected in 50 cases by DCE-MRI,49 cases by DWI,and 54 cases by DCE-MRI combined with DWI.The sensitivity and accuracy of combined examination was higher than that of DCE-MRI and DWI alone,and the differences were statistically significant(both P<0.05).And the kappa value was 0.940 of combined examination consistent with the gold standard.The receiver operating characteristic curve showed that the area under the curve of DCE-MRI,DWI alone and combined diagnosis of breast cancer were all greater than 0.70,and the combined diagnosis value of DCE-MRI and DWI was the highest.Conclusion The combined application of DCE-MRI and DWI can significantly improve the diagnostic efficacy of breast cancer,which is consistent with the results of pathological section examination.
