彩色多普勒超声在急性输尿管结石诊断中应用的价值

Value of Color Doppler Ultrasound in the Diagnosis of Acute Ureteral Stones

目的分析彩色多普勒超声在急性输尿管结石诊断中的应用价值。方法选取2020年1月—2023年9月徐州淮海国际港务区人民医院超声科收治的131例疑似急性输尿管结石患者,分别进行二维超声、彩色多普勒超声检查,以患者尿道排出结石及外科手术的结果作为金标准,比较两种检查方式的诊断符合率及诊断效能(灵敏度、特异度、准确度)。结果在急性输尿管诊断中,彩色多普勒超声阳性检出率高于二维超声,彩色多普勒超声诊断的灵敏度为94.83%(110/116)、特异度为80.00%(12/15)和准确度为93.13%(122/131),均高于二维超声的65.52%(76/116)、40.00%(6/15)、62.60%(82/131),差异有统计学意义(χ^(2)=31.345、5.000、4.442,P均<0.05);彩色多普勒超声对上中下段结石的诊断符合率均高于二维超声,差异有统计学意义(P均<0.05)。结论在急性输尿管结石诊断中,彩色多普勒超声具有较高的诊断价值。

Objective To analyze the application value of color Doppler ultrasonic in the diagnosis of acute ureteral stones.Methods From January 2020 to September 2023,131 patients with suspected acute ureteral calculi who underwent ultrasound examination in the Ultrasound Department of People's Hospital of Huaihai International Port Area were selected for two-dimensional ultrasound and color Doppler ultrasound examination.The results of patients'urethral discharge stones and surgery were used as the gold standard to compare the diagnostic coincidence rate and diagnostic efficacy(sensitivity,specificity,accuracy)of the two methods in the disease.Results In the diagnosis of acute ureter,the positive detection rate of color Doppler ultrasound was higher than that of two-dimensional ultrasound.The sensitivity was 94.83%(110/116),specificity was 80.00%(12/15)and accuracy was 93.13%(122/131)of color Doppler ultrasound were higher than those of twodimensional ultrasound 65.52%(76/116),40.00%(6/15),62.60%(82/131),and the differences were statistically significant(χ^(2)=31.345,5.000,4.442,all P<0.05).The diagnostic coincidence rate of color doppler ultrasound in upper,middle and lower stones were higher than that of two-dimensional ultrasound,and the difference were statistically significant(all P<0.05).Conclusion In the diagnosis of acute ureteral stones,color Doppler ultrasound has a high diagnostic value.
