血液灌注联合血液透析对糖尿病肾病晚期血液透析患者血糖水平的影响

摘要 目的分析血液灌注联合血液透析对糖尿病肾病晚期血液透析患者血糖水平的影响。方法选择2022年2月—2024年3月福建省德化县医院收治的102例糖尿病肾病晚期血液透析患者为研究对象,根据治疗方式不同分为对照组(n=51)、研究组(n=51)。对照组实施血液透析治疗,研究组在对照组基础上实施血液灌注。比较两组血糖水平、肾功能、炎症因子水平。结果治疗后,研究组空腹血糖、餐后2 h血糖、糖化血红蛋白水平均低于对照组,差异有统计学意义(P均<0.05)。治疗后,研究组24 h尿蛋白定量、血肌酐、总蛋白、尿素氮、血清白蛋白水平均优于对照组,差异有统计学意义(P均<0.05)。治疗后,研究组超敏C反应蛋白、白细胞介素-6、降钙素原水平均低于对照组,差异有统计学意义(P均<0.05)。结论血液灌注联合血液透析能够降低糖尿病肾病晚期血液透析患者血糖水平、改善患者肾功能并降低炎症因子水平,有助于延缓病情进展、改善患者预后。 Objective To analyze the effect of blood perfusion combined with hemodialysis on blood glucose level in patients with advanced hemodialysis of diabetic nephropathy.Methods A total of 102 patients with advanced hemodialysis of diabetic nephropathy admitted to Dehua County Hospital of Fujian Province from February 2022 to March 2024 were selected as the study objects and divided into control group(n=51)and study group(n=51)according to different treatment methods.The control group was treated with hemodialysis,and the study group was treated with blood perfusion on the basis of the control group.The levels of blood glucose,renal function and inflammatory factors were compared between the two groups.Results After treatment,the levels of fasting blood glucose,2-hour postprandial blood glucose and glycated hemoglobin A1c in the study group were lower than those in the control group,and the differences were statistically significant(all P<0.05).After treatment,the levels of 24-hour urinary protein quantity,serum creatinine,total protein,urea nitrogen and serum albumin in the study group were better than those in the control group,and the differences were statistically significant(all P<0.05).After treatment,the levels of high-sensitivity C-reactive protein,interleukin-6 and procalcitonin in the study group were lower than those in the control group,and the differences were statistically significant(all P<0.05).Conclusion Blood perfusion combined with hemodialysis can reduce the blood glucose level,improve the renal function and reduce the level of inflammatory factors in patients with advanced hemodialysis of diabetic nephropathy,which is helpful to delay the progression of the disease and improve the prognosis of patients.
