精液中白细胞浓度与精液质量及男性生育能力的相关性探讨

Correlation Exploriation Between White Blood Cell Concentration in Semen,Semen Quality and Male Fertility

目的研究分析精液中白细胞浓度与精液质量及男性生育能力的相关性。方法选取2020年1月—2023年6月在本院接受治疗的100名患有男性不育症的患者,以及100名生育能力正常的男性健康体检者作为研究对象,分别将其划分为研究组和对照组。在两组受检者中采集精液样本开展检测,对精液质量各项指标进行检测,再对精液中的白细胞浓度进行检测。对比研究组与对照组的精液质量指标、精液白细胞浓度。根据男性不育症患者的精液白细胞浓度,以1×10^(6)/mL为临界值,将男性不育症患者分为异常组(精液白细胞浓度>1×10^(6)/mL)、正常组(精液白细胞浓度≤1×10^(6)/mL),对比异常组与正常组的精液质量指标,分析精液白细胞浓度与精液质量各项指标、男性生育能力之间的相关性。结果研究组的精液白细胞浓度高于对照组(P<0.05)。研究组精液样本中的精子密度、精子存活率、精子活动率、精子形态正常率均低于对照组(P<0.05)。以1×10^(6)/mL为临界值,精液白细胞浓度异常组精液样本中的精子密度、精子存活率、精子活动率、精子形态正常率均低于正常组(P<0.05)。经相关性分析,精液白细胞浓度与精液质量指标精子密度、精子存活率、精子活动率、精子形态正常率之间均呈负相关,精液白细胞浓度与男性生育能力之间同样呈负相关。结论男性不育症患者精液白细胞浓度普遍出现异常增高情况,其精液质量明显下降,精液白细胞浓度与男性精液质量及生育能力均存在密切关联。

Objective To study and analyze correlation between white blood cell concentration in semen,semen quality and male fertility.Methods The paper chose 100 patients with male infertility with medical treatment in the hospital from January 2020 to June 2023 and 100 healthy male individuals with normal fertility as study samples,and treated them as study group and control group respectively.Semen samples were collected from two groups for testing of various semen quality indicators,and concentration of white blood cells in male semen.Semen quality indicators and semen leukocyte concentration were compared between study group and control group.Based on semen white blood cell concentration of male infertility patients,with critical value of 1×10^(6)/mL,male infertility patients were divided into abnormal group(with semen white blood cell concentration>1×10^(6)/mL)and normal group(with semen white blood cell concentration≤1×10^(6)/mL).Semen quality indicators of abnormal group and normal group were compared,correlation between semen white blood cell concentration,semen quality indicators and male fertility was analyzed.Results Concentration of white blood cells in semen of study group was higher than control group(P<0.05),while sperm density,sperm survival rate,sperm motility rate and normal sperm morphology rate in semen samples of study group was lower than control group(P<0.05).At critical value of 1×10^(6)/mL,sperm density,sperm survival rate,sperm motility rate and normal sperm morphology rate in semen samples of the group with abnormal semen white blood cell concentration was lower than normal group(P<0.05).According to correlation analysis,there was negative correlation between semen white blood cell concentration and semen quality indicators of sperm density,sperm survival rate,sperm motility rate and normal sperm morphology.Similarly,there was negative correlation between semen white blood cell concentration and male fertility.Conclusion Male infertility patients generally experience an abnormal increase of white blood cell concentration in semen,which causes significant decrease of semen quality.Concentration of white blood cells in semen is closely related to male semen quality and fertility.
