彩色多普勒超声诊断四肢血管栓塞的临床意义

Clinical Significance Research of Color Doppler Ultrasound in Diagnosis of Limb Vascular Embolism

目的探讨彩色多普勒超声诊断四肢血管栓塞的临床意义。方法选取2022年1月—2022年12月本院收治的83例疑似四肢血管栓塞患者作为研究对象,对其均进行彩色多普勒超声检查,以血管造影检查结果作为金标准,分析彩色多普勒超声的诊断效果、诊断效能,对比四肢血管栓塞和非四肢血管栓塞患者的血管内径。结果彩色多普勒超声对四肢血管栓塞的诊断敏感度为98.36%(60/61)、特异度为100.00%(22/22)、准确度为98.80%(82/83)、阳性预测值为100.00%(60/60)、阴性预测值为95.65%(22/23);61例四肢血管栓塞患者中,上肢血管栓塞12例,下肢血管栓塞49例;四肢血管栓塞患者股总静脉、股浅静脉、周围静脉近端、内侧腓肠静脉的内径均大于非血管栓塞患者(P<0.05),但两组腘静脉内径相当(P>0.05)。结论彩色多普勒超声诊断四肢血管栓塞有重要临床意义,诊断效能高,能有效明确栓塞部位、管径,显示特征性栓塞征象。

Objective To explore clinical significance of color Doppler ultrasound in diagnosis of limb vascular embolism.Methods The paper chose 83 suspected patients with limb vascular embolism in our hospital from January 2022 to December 2022 as research subjects,treated with color Doppler ultrasound examination.Diagnostic effect and efficiency of color Doppler ultrasound was analyzed with gold standard of angiography,and vascular diameter of patients with limb vascular embolism and those without limb vascular embolism was compared.Results Diagnostic sensitivity of color Doppler ultrasound for limb vascular embolism was 98.36%(60/61),specificity was 100.00%(22/22),accuracy was 98.80%(82/83),positive predictive value was 100.00%(60/60),and negative predictive value was 95.65%(22/23).Among 61 patients with limb vascular embolism,there were 12 cases of upper limb vascular embolism,49 cases of lower limb vascular embolism.In ner diameters of common femoral vein,superficial femoral vein,proximal peripheral vein and medial gastrocnemius vein of patients with limb vascular embolism were larger than non thrombotic patients(P<0.05),but inner diameters of popliteal vein were similar between two groups(P>0.05).Conclusion Color Doppler ultrasound has important clinical significance in diagnosis of limb vascular embolism with high diagnostic efficiency,can clarify location and diameter,display characteristic signs of the embolism effectively.
