小剂量右美托咪定对子宫下段剖宫产术中寒战和牵拉反应的影响

Effect of low-dose Dexmedetomidine on shivering and traction reaction during low segment cesarean section

目的探讨小剂量右美托咪定对子宫下段剖宫产术中寒战和牵拉反应的影响。方法选取2022年2月至2024年2月于广东祈福医院行腰硬联合麻醉下子宫下段剖宫产术的60例产妇作回顾性分析,根据胎儿娩出后有无静脉注射右美托咪定分为试验组与对照组,每组各30例。试验组在胎儿娩出后静脉注射右美托咪定0.25μg/kg,注射时间>5min,对照组注射等量生理盐水。比较两组寒战、牵拉反应及不良反应发生情况。结果试验组寒战总发生率低于对照组(P<0.05)。试验组牵拉反应优良率高于对照组(P<0.05)。两组均未发生心动过缓、低血压等不良反应。结论小剂量右美托咪定能降低子宫下段剖宫产术中寒战的发生率,减轻牵拉反应,且不增加不良反应。

Objective To investigate the effect of low-dose Dexmedetomidine on shivering and traction reaction during low segment cesarean section.Methods A retrospective analysis was conducted on 60 parturients undergoing lower segment cesarean section under combined spinal and epidural anesthesia in Guangdong Clifford Hospital from February 2022 to February 2024.All the participants were divided into the experimental group and the control group based on whether Dexmedetomidine was intravenously administered after fetal delivery,with 30 cases in each group.The experimental group received intravenous of 0.25μg/kg Dexmedetomidine after fetal delivery,with an injection time of>5 minutes,while the control group received an equal volume of physiological saline injection.The incidence of shivering,traction reaction,and adverse reactions were compared between the two groups.Results The total incidence of shivering in the experimental group was lower than that in the control group(P<0.05).The excellent rate of traction reaction in the experimental group was higher than that in the control group(P<0.05).No adverse reactions such as bradycardia or hypotension occurred in either group.Conclusion Low-dose Dexmedetomidine in lower segment cesarean section can reduce the incidence of shivering and alleviate tra ction reaction without increasing adverse reactions.
