术前碘伏阴道擦洗联合术中双氧水冲洗宫腔预防胎膜早破剖宫产术后产褥感染的效果

Effect of preoperative iodine vaginal scrubbing combined with intraoperative hydrogen peroxide flushing of the uterine cavity on preventing postpartum infection after cesarean section with premature rupture of membranes

目的:探究术前碘伏阴道擦洗联合术中双氧水冲洗宫腔预防胎膜早破剖宫产术后产褥感染的效果。方法:选取2023年1月到2023年12月我院收治的60例胎膜早破行剖宫产患者60例作为研究对象,随机分为对照组和研究组各30例,对照组给予抗生素治疗,研究组给予抗生素治疗+术前碘伏阴道擦洗联合术中双氧水冲洗宫腔。比较两组治疗前后的血清C-反应蛋白(C-reactive protein,CRP)、白介素-6(Interleukin-6,IL-6)、降钙素原(Procalcitonin,PCT)水平、白细胞(White blood cell,WBC)计数、中性粒细胞(Neutrophils,NEUT)比例及术后产褥感染的发生率;比较两组患者的住院时间、平均体温及术后生活质量评分(生理机能、生理功能、社会功能、总体健康)。结果:术后24 h,研究组CRP、PCT、WBC、NEUT水平低于对照组,IL-6水平高于对照组(P<0.05);研究组患者的术后产褥感染的发生率、平均体温、住院时间及均低于对照组(P<0.05);研究组患者的生理机能、生理功能、社会功能、总体健康评分均高于对照组(P<0.05)。结论:术前碘伏阴道擦洗联合术中双氧水冲洗宫腔预防胎膜早破剖宫产术后产褥感染的临床效果显著,有效降低术后产褥感染的发生率,提高患者的生活质量,降低炎症因子的水平,而且有效的降低患者的住院时间,值得临床推广使用。

Objective:To investigate the effect of preoperative iodine vaginal scrubbing combined with intraoperative hydrogen peroxide flushing of the uterine cavity on preventing postpartum infection after cesarean section with premature rupture of membranes.Method:Sixty patients with premature rupture of membranes undergoing cesarean section admitted to our hospital from January 2023 to December 2023 were selected as the study subjects.They were randomly divided into control group and study group,with 30 patients in each group.The control group received antibiotic treatment,while the study group received antibiotic treatment combined with preoperative iodine swabbing of the vaginal canal and intraoperative hydrogen peroxide flushing of the uterine cavity.The levels of serum C-reactive protein(CRP),interleukin-6(IL-6),procalcitonin(PCT),white blood cell(WBC)count,neutrophil(NEUT)ratio,and incidence of postpartum infection were compared between two groups before and after treatment;The time of hospital stay,average body temperature and the postoperative quality of life scores(physiological function,physiological function,social function,overall health)were compared between two groups of patients;Compare between two groups of patients using a self-made survey questionnaire developed by our hospital.Result:24 hours after surgery,the levels of CRP,PCT,WBC,and NEUT in the study group were lower than those in the control group,while the level of IL-6 was higher than that in the control group(P<0.05);The incidence of postpartum infection,average body temperature,and time of hospital stay in the study group were lower than those in the control group(P<0.05);The physiological function,physiological function,social function,and overall health score of the study group patients were higher than those of the control group(P<0.05).Conclusion:Preoperative iodine vaginal scrubbing combined with intraoperative hydrogen peroxide flushing of the uterine cavity has a significant clinical effect in preventing postpartum infection after premature rupture of membranes and cesarean section.It effectively reduces the incidence of postpartum infection,improves the quality of life of patients,reduces the level of inflammatory factors,and effectively reduces the time of hospital stay.It is worthy of clinical promotion and use.
