摘要
目的:探讨尿微量白蛋白(Microalbumin,mAlb)、尿转铁蛋白(Transferrin,TRF)及醛固酮(Aldosterone,Ald)在糖尿病肾病(Diabetic nephropathy,DN)诊断、损害评估中的作用。方法:回顾性收集2023年2月至2023年12月期间本院收治的98例DN患者的临床资料,将其纳入DN组。同时选取同期本院健康者体检者54例,设为对照组。根据肾小球滤过率(Glomerular filtration rate,GFR)将DN患者分为早期DN组与中晚期DN组。分析比较各组的mAlb、TRF及Ald水平;采用Pearson相关性分析mAlb、TRF及Ald与GFR的相关性。并绘制受试者工作特征(Receiver operating characteristic,ROC)曲线分析mAlb、TRF及Ald检测对DN的诊断价值。结果:DN组mAlb、TRF水平均显著高于对照组,Ald水平显著低于对照组(P<0.05)。早期DN组的mAlb、TRF水平均显著低于中晚期DN组,Ald水平显著高于中晚期DN组(P<0.05)。血清mAlb、TRF与GFR呈负相关关系,Ald与GFR呈正相关关系(P<0.05)。ROC曲线示,三者联合(mAlb+TRF+Ald)联合检测诊断DN的敏感度和特异度分别为0.918、0.817;AUC值=0.900,显著高于各项指标单独诊断(P<0.05)。结论:mAlb、TRF及Ald可作为诊断DN、评估肾损伤的重要指标,三者联合检测能进一步提高对DN的诊断效果。
Objective:To investigate the role of urinary Microalbumin(mAlb),Transferrin(TRF)and Aldosterone(Ald)in the diagnosis and lesion assessment of Diabetic nephropathy(DN).Methods:The clinical data of 98 patients with DN admitted to our hospital from February 2023 to December 2023 were retrospectively collected and included into the DN group.At the same time,54 healthy subjects in our hospital were selected as the control group.According to Glomerular filtration rate(GFR),DN patients were divided into early DN group and middle and late DN group.mAlb,TRF and Ald levels of all groups were analyzed and compared.The correlations between mAlb,TRF and Ald and GFR were analyzed by Pearson correlation.Receiver operating characteristic(ROC)curves were drawn to analyze the diagnostic value of mAlb,TRF and Ald detection in DN.Results:The levels of mAlb and TRF in DN group were significantly higher than those in control group,and the levels of Ald were significantly lower than those in control group(P<0.05).mAlb and TRF levels in early DN group were significantly lower than those in middle and late DN group,and Ald level was significantly higher than that in middle and late DN group(P<0.05).Serum mAlb and TRF were negatively correlated with GFR,while Ald was positively correlated with GFR(P<0.05).ROC curve showed that the sensitivity and specificity of the combined detection of DN by mAlb+TRF+Ald were 0.918 and 0.817,respectively.The value of AUC=0.900 was significantly higher than that of each index alone(P<0.05).Conclusion:mAlb,TRF and Ald can be used as important indicators for diagnosis of DN and evaluation of renal injury,and the combined detection of MALB,TRF and ALD can further improve the diagnostic effect of DN.
