摘要
目的:探究影响急性缺血性脑卒中(Acute Ischemic Stroke,AIS)静脉溶栓治疗预后的相关因素。方法:选取2018年7月至2022年7月间本院收治的96例AIS患者作为研究对象,回顾性分析所有患者临床及随访资料、胱抑素C(Cyata tin C,Cys C)和血糖水平及预后情况。根据患者的预后不同将其分为预后良好组(61例)和预后不良组(35例),探究影响AIS患者静脉溶栓预后的危险因素。结果:预后良好组的美国国立卫生研究院卒中量表(NIH Stroke Scale,NIHSS)评分、空腹血糖、空腹血压及Cys C水平均明显低于预后不良组,预后良好组糖化血红蛋白(Hemoglobin A1c,HbA1c)水平6.5~10%及>10%占比均明显低于预后不良患组(P<0.05)。HbA1c水平及Cys C水平均与AIS患者静脉溶栓预后不良有关,NIHSS评分越高,其不良预后风险也增高。结论:糖化血红蛋白、胱抑素C水平升高与AIS患者静脉溶栓不良预后相关。
Objective:To explore the related factors affecting the prognosis of acute ischemic stroke(AIS)treated with intravenous thrombolysis.Methods:96 patients with AIS admitted to our hospital from July 2018 to July 2022 were selected as the study objects.The clinical and follow-up data,Cystatin C(Cys C),blood glucose levels,and prognosis based on the data were retrospectively analyzed.Based on prognosis,they were divided into good prognosis(61 cases)and bad prognosis(35 cases).The risk factors affecting the prognosis of intravenous thrombolysis in patients with AIS were explored.Results:Among 96 patients with AIS,61 had good prognosis.35 patients with poor prognosis.The National Institutes of Health Stroke Scale(NIHSS)score,fasting blood glucose,fasting blood pressure and Cys C levels in patients with good prognosis were significantly lower than those in patients with poor prognosis(P<0.05).The proportion of glycated hemoglobin levels between 6.5-10%and>10%in patients with good prognosis was significantly lower than that in patients with poor prognosis(P<0.05).Both HbA1c and Cys C levels were associated with poor prognosis in AIS patients with intravenous thrombolysis,and the higher NIHSS score,the higher risk of poor prognosis.Conclusion:Increased levels of glycosylated hemoglobin and cystatin C are associated with poor prognosis in AIS patients.
