不同质子泵抑制剂对Hp感染胃溃疡的治疗效果及对白细胞介素-33、miR-26a水平的影响

The Therapeutic Effect of Different Proton Pump Inhibitors on Hp Infected Gastric Ulcers and Their Impact on Interleukin-33 and miR-26a Levels

目的探讨不同质子泵抑制剂对幽门螺杆菌(Hp)感染胃溃疡的治疗效果及对白细胞介素-33(IL-33)、miR-26a水平影响。方法将2022年6月—2023年6月西安市临潼区人民医院收治的86例Hp感染胃溃疡患者作为研究对象,利用随机数表法分成对照组和观察组,每组43例。给予对照组质子泵抑制剂(奥美拉唑肠溶片)+胶体果胶铋胶囊+克拉霉素片+甲硝唑片四联治疗方案,给予观察组质子泵抑制剂(埃索美拉唑镁)+胶体果胶铋胶囊+克拉霉素片+甲硝唑片四联治疗方案,比较两组临床治疗总有效率、Hp根除率、IL-33水平、miR-26a水平、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)。结果观察组治疗总疗效高于对照组,差异具有统计学意义(P<0.05);观察组Hp根除率高于对照组,差异具有统计学意义(P<0.05)。经治疗,两组IL-33水平均明显降低,而miR-26a水平均明显提高,且观察组IL-33水平显著低于对照组,miR-26a水平显著高于对照组(P<0.05);经治疗,两组CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)水平均明显改善,且观察组CD4^(+)水平以及CD4^(+)/CD8^(+)比值显著高于对照组,CD8^(+)水平显著低于对照组(P<0.05)。结论相比于奥美拉唑肠溶片,埃索美拉唑镁可提高患者临床治疗效果和Hp根除率,降低患者IL-33水平、提高miR-26a水平,提高患者免疫功能。

Objective To explore the therapeutic effect of different proton pump inhibitors on Helicobacter pylori(Hp)infected gastric ulcers and their impact on interleukin-33 and miR-26a levels.Methods A study was conducted on 86 patients with Hp infected gastric ulcers admitted to the People's Hospital of Lintong District,Xi'an City from June 2022 to June 2023.Divide into two groups using the random number table method,with 43 cases in each group.The control group was given a proton pump inhibitor(omeprazole enteric coated tablets)+colloidal pectin bismuth capsules+clarithromycin tablets+metronidazole tablets quadruple treatment plan,while the observation group was given a proton pump inhibitor(esomeprazole magnesium)+colloidal pectin bismuth capsules+clarithromycin tablets+metronidazole tablets quadruple treatment plan.Compare the total effective rate,Hp eradication rate,the levels of interleukin-33,miR-26a,CD4^(+),CD8^(+),CD4^(+)/CD8^(+)between two groups.Results The total therapeutic effect of the observation group was higher than that of the control group,with statistical significance(P<0.05);The Hp eradication rate in the observation group was higher than that of the control group,with statistical significance(P<0.05).After treatment,the levels of IL-33 in both groups were significantly reduced,while the levels of miR-26a were significantly increased.The level of IL-33 s in the observation group was lower than that in the control group,while the level of miR-26a was higher than that in the control group(P<0.05).After treatment,the levels of CD4^(+)and CD8^(+)in both groups showed significant improvement,with the former showing an increase and the latter showing a decrease.The level of CD4^(+)and the ratio of CD4^(+)/CD8^(+)in the observation group were higher than those in the control group,while the level of CD8^(+)was lower than those in the control group(P<0.05).Conclusion Compared to omeprazole enteric coated tablets,esomeprazole magnesium can improve clinical treatment efficacy and Hp eradication rate in patients,reduce the level of interleukin-33,increase the level of miR-26a,and improve immune function.
