中西医结合护理对预防老年股骨头置换术后静脉血栓栓塞症的效果

The effect of Integrated Traditional Chinese and Western Medicine Nursing on Preventing Venous Thromboembolism after Elderly Femoral Head Replacement Surgery

目的研究老年股骨头置换手术患者采用中西医结合护理对于术后静脉血栓栓塞症(VTE)的预防效果。方法选取2021年5月—2023年11月泰州市中西医结合医院收治的股骨头置换手术老年患者88例作为研究对象,以奇偶数字法将患者分为对照组和研究组,每组44例。对照组仅开展常规护理,研究组于对照组基础联合开展中医护理方案,针对两组术后凝血功能指标、肢体肿胀发生情况、生活质量、护理满意度进行对比观察。结果护理后,研究组血浆纤维蛋白原(FIB)、纤维蛋白降解物(FDP)、D-二聚体(D-D)三项指标水平均显著低于对照组,差异有统计学意义(P<0.05);研究组肢体肿胀发生率显著低于对照组,差异有统计学意义(P<0.05);研究组生理功能、心理功能以及躯体功能评分均高于对照组,差异有统计学意义(P<0.05);研究组VTE评估、VTE预防、服务态度以及专业技能三项评分均显著高于对照组,差异有统计学意义(P<0.05)。结论针对接受股骨头置换手术的老年患者,对其开展中西医结合护理可有效促进患者凝血功能恢复,降低VTE发生风险,患者术后生活质量水平更高,患者满意度高,临床应用价值良好。

Objective To study the preventive effect of integrated traditional Chinese and Western medicine nursing on postoperative venous thromboembolism(VTE)in elderly patients undergoing femoral head replacement surgery.Methods From May 2021 to November 2023,88 elderly patients who underwent femoral head replacement surgery at the Taizhou Integrated Traditional Chinese and Western Medicine Hospital were selected as the study subjects.The patients were divided into groups using odd and even numbers.The control group(n=44)received only routine care,while the study group(n=44)received a combined traditional Chinese medicine nursing plan on the basis of the control group,targeting postoperative coagulation function indicators in both groups Compare and observe the occurrence of limb swelling,quality of life,and nursing satisfaction.Results After nursing,compared with the control group,the levels of plasmafibrinogen(FIB),fibrinolytic products(FDP),and D-dimer(D-D)in the study group were all lower than those in the control group,with statistical significance(P<0.05);The incidence of limb swelling in the study group was lower than the control group,and the difference was statistically significant(P<0.05);Comparing the scores of physiological,psychological,and physical functions between the two groups,the study group had higher scores than the control group,and the difference was statistically significant(P<0.05);Comparing the satisfaction levels of the two groups,the study group showed higher scores in VTE assessment,VTE prevention,service attitude,and professional skills compared to the control group,with statistically significant differences(P<0.05).Conclusion For elderly patients undergoing femoral head replacement surgery,integrated traditional Chinese and Western medicine nursing can effectively promote the recovery of coagulation function,reduce the risk of VTE,and improve their postoperative quality of life,which has received high satisfaction from patients and has good clinical application value.
