邻面边缘提升术在龈下缺损高嵌体修复中的应用

Application of proximal edge lifting in the onlay restoration of subgingival defects

目的:分析邻面边缘提升术在龈下缺损高嵌体修复中的应用效果。方法:选取本院2022年1月至11月收治的113例龈下缺损患者,每例患者选取1颗患牙。采用简单随机数字表法将患者分为对照组(n=56,常规备牙高嵌体修复)与观察组(n=57,邻面边缘提升术后高嵌体修复)。比较两组修复效果、咀嚼功能、牙龈情况及患者满意度。结果:修复后6个月、12个月时,两组修复体折裂、修复体固位、边缘着色、边缘密合性、牙体完整性及继发龋情况比较,差异无统计学意义(P>0.05);修复后12个月内,观察组未发生继发龋,出现修复体折裂B级2例(3.51%)、边缘着色B级3例(5.26%)、牙体完整性B级1例(1.75%);对照组出现B级继发龋3例,出现食物嵌塞1例。修复后6个月、12个月时,观察组咀嚼效率高于对照组,GI、PLI值低于对照组(P<0.05)。观察组总满意度高于对照组(P<0.05)。结论:邻面边缘提升术在龈下缺损高嵌体修复中的临床修复效果良好,可改善患者咀嚼功能、牙龈状况,满意度高。

Objective:To evaluate the effectiveness of proximal edge lifting in the onlay restoration of subgingival defects.Methods:A total of 113 patients with subgingival defects admitted to our hospital from January 2022 to November 2022 were selected,with 1 affected tooth selected for each patient.Patients were divided into control group(n=56,conventional onlay restoration)and observation group(n=57,onlay restoration after proximal edge lifting)using a simple random number table method.Restoration effect,chewing function,gingival condition and patients’satisfaction were compared between the two groups.Results:6 months and 12 months after restoration,there were no significant differences in fracture,retention,edge coloring,edge adhesion,integrity and secondary caries between the two groups(P>0.05).Within 12 months after restoration,there was no secondary caries in the observation group.There were 2 cases of fracture B(3.51%),3 cases of marginal staining B(5.26%),and 1 case of dental integrity B(1.75%).In the control group,there were 3 cases of grade B secondary caries and 1 case of food Mosaic.At the 6th and 12th months after restoration,the masticatory efficiency of the observation group was higher than that of the control group,and the GI and PLI values were lower than those of the control group(P<0.05).The total satisfaction of the observation group was higher than that of the control group(P<0.05).Conclusion:The proximal edge lifting technique has a good clinical effect in the onlay restoration of subgingival defect,which can improve the masticatory function and gingival condition of patients with high satisfaction.
