连续性肾脏替代治疗+血液灌流(CRRT+HP)联合中药治疗高甘油三酯血症型重症急性胰腺炎的临床应用效果研究

Clinical Application Effect of Continuous Renal Replacement Therapy+Hemoperfusion(CRRT+HP)Combined with Traditional Chinese Medicine in Treatment of Severe Acute Pancreatitis with Hypertriglyceridemia

目的 分析连续性肾脏替代治疗+血液灌流(CRRT+HP)联合中药治疗高甘油三酯血症型重症急性胰腺炎(HLSAP)的应用效果。方法 选取2020年1月—2023年12月本院收治的40例HLSAP患者开展研究,用随机数字表法分为对照和观察组,每组20例。其中,对照组行CRRT+清胰汤治疗,观察组行CRRT+HP+清胰汤治疗,比较两组临床疗效。结果 观察组的治疗有效率明显高于对照组(P<0.05);观察组复发率、腹腔内压力、PCT、IL-6、脂肪肝影像学评分、APACHEⅡ评分、AST、ALT和TG均明显低于对照组(P<0.05)。结论 针对HLSAP患者的治疗,采用CRRT+HP+清胰汤的综合治疗方案,已被证实能够显著减轻患者病情,有效调节血清指标,治疗效果明确且显著。鉴于其明确的临床效果,该治疗方案值得广泛应用。

Objective To analyze application effect of CRRT+HP traditional Chinese medicine in treatment of severe acute pancreatitis(HLSAP)with hypertriglyceridemia.Methods The paper chose 40 patients with HLSAP in our hospital from January 2020 to December 2023,and randomly divided them into control group of 20 cases,treated with CRRT+pancreas clearance decoction,and observation group of 20 cases,with CRRT+HP+pancreas clearance decoction.Clinical effect between two groups was compared.Results Curative effective rate of observation group was significantly higher than control group,and recurrence rate,intra-abdominal pressure,PCT,IL-6,fatty liver imaging scores,APACHEⅡscores,AST,ALT,and TG were significantly lower than control group(P<0.05).Conclusion CRRT+HP+pancreas clearance decoction can alleviate condition of HLSAP patients effectively,regulate serum indicators with clear effect,which has promotional value.
