基于AIDET沟通模式的健康教育对急性白血病患者负性情绪及生活质量的影响

摘要 目的探讨基于问候-自我介绍-过程-解释-感谢(AIDET)沟通模式的健康教育对急性白血病(AL)患者负性情绪及生活质量的影响。方法将98例AL患者依据干预方式的不同分为AIDET组(52例,基于AIDET沟通模式的健康教育)和参照组(46例,常规健康教育)。比较两组患者负性情绪[汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)评分]、生活质量[世界卫生组织生存质量测定量表简表(WHOQOL-BREF)评分]、不良反应发生情况。结果干预后,两组患者HAMA、HAMD量表评分均较干预前降低,且AIDET组患者HAMA、HAMD量表评分均低于参照组,差异均有统计学意义(P<0.05)。干预后,两组患者社会关系、生理方面、心理方面及环境方面评分均较干预前升高,且AIDET组患者社会关系、生理方面、心理方面及环境方面评分均高于参照组,差异均有统计学意义(P<0.05)。AIDET组患者骨髓抑制、呕吐、肝功能受损、口腔黏膜炎发生率均低于参照组,差异均有统计学意义(P<0.05)。结论基于AIDET沟通模式的健康教育能够显著改善AL患者负性情绪和生活质量,降低不良反应发生率。 Objective To explore the effects of health education based on acknowledge-introduce-duration-explanationthank(AIDET)communication model on negative emotions and quality of life in patients with acute leukemia(AL).Method A total of 98 AL patients were divided into AIDET group(52 cases,health education based on AIDET communication model)and reference group(46 cases,conventional health education)according to different intervention methods.Negative emotions[Hamilton anxiety scale(HAMA),Hamilton depression scale(HAMD)scores],quality of life[World Health Organization quality of life brief(WHOQOL-BREF)score]and adverse reactions were compared between the two groups.Result After intervention,the scores of HAMA and HAMD scales in both groups were lower than those before intervention,and the scores of HAMA and HAMD scales in AIDET group were lower than those in reference group,the differences were statistically significant(P<0.05).After intervention,the scores of social relations,physiological aspects,psychological aspects and environmental aspects in both groups were higher than those before intervention,and the scores of social relations,physiological aspects,psychological aspects and environmental aspects in AIDET group were higher than those in reference group,the differences were statistically significant(P<0.05).The incidences of myelo-suppression,vomiting,liver function impairment and oral mucositis in AIDET group were lower than those in reference group,the differences were statistically significant(P<0.05).Conclusion Health education based on AIDET communication model can significantly improve the negative emotions and quality of life of AL patients,and reduce the incidence of adverse reactions.
