摘要
目的探讨基于叙事理论的健康宣教对多发性骨髓瘤(MM)患者心理状态及治疗依从性的影响。方法根据干预方法的不同将86例MM化疗患者分为对照组(n=44,常规干预)和观察组(n=42,基于叙事理论的健康宣教)。比较两组患者的心理状态[正性负性情绪量表(PAN AS)]、治疗依从性(遵医行为调查问卷)、不良反应发生情况及满意度。结果干预3周后,两组患者正性情绪评分均高于本组干预前,负性情绪评分均低于本组干预前,观察组患者正性情绪评分高于对照组,负性情绪评分低于对照组,差异均有统计学意义(P<0.05)。观察组患者药物服用、运动饮食、日常管理、定期复查评分均明显高于对照组,差异均有统计学意义(P<0.01)。观察组患者的不良反应总发生率低于对照组,满意度优于对照组,差异均有统计学意义(P<0.05)。结论基于叙事理论的健康宣教可改善MM化疗患者的心理状态,提高治疗依从性和满意度,降低不良反应发生率。
Objective To explore the effect of health education based on narrative theory on mental state and treatment compliance of multiple myeloma(MM)patients undergoing chemotherapy.Method According to different intervention methods,86 patients with MM who underwent chemotherapy were divided into control group(n=44,conventional intervention)and observation group(n=42,health education based on narrative theory).The mental state[positive and negative affect schedule(PANAS)],treatment compliance(compliance behavior questionnaire),adverse reactions and sat-isfaction were compared between the two groups.Result After 3 weeks of intervention,the positive emotion scores in both groups were higher than those before intervention,and the negative emotion scores were lower than those before intervention,and the positive emotion score in the observation group was higher than that in the control group,the negative emotion score was lower than that in the control group,and the differences were statistically significant(P<0.05).The scores of drug consumption,exercise diet,daily management and periodic review in observation group were significantly higher than those in control group,and the differences were statistically significant(P<0.01).The total incidence of adverse reactions in observation group was lower than that in control group,and the satisfaction rate was better than that in control group,and the differences were statistically significant(P<0.05).Conclusion Health education based on narrative theory can improve the mental state of MM patients undergoing chemotherapy,improve the treatment compliance and satisfaction,and reduce the incidence of adverse reactions.
