高频胸部振荡治疗老年重症肺炎的效果分析

摘要 目的:分析高频胸部振荡(HFCWO)治疗老年重症肺炎的效果。方法:选取2020年10月—2024年1月榆林市靖边县人民医院收治的100例老年重症肺炎患者,随机分为对照组和观察组,每组50例。对照组在常规治疗基础上接受纤支镜肺泡灌洗治疗,观察组在对照组基础上给予HFCWO治疗。比较两组临床疗效、临床症状体征消失时间、血气指标、血清C反应蛋白(CRP)及降钙素原(PCT)水平。结果:观察组总有效率高于对照组(P=0.037);观察组湿啰音消失、退热、咳嗽消退、心力衰竭改善、气促消失时间均短于对照组(P<0.001);治疗后,两组动脉二氧化碳分压较治疗前降低,动脉血氧分压、氧合指数较治疗前升高,观察组优于对照组(P<0.05);治疗后,两组血清CRP、PCT水平较治疗前降低,观察组低于对照组(P<0.05)。结论:HFCWO治疗老年重症肺炎的效果显著,可有效促进患者临床症状消退,调节血气指标,减轻炎性反应。 Objective:To analyze the effect of high frequency chest oscillation(HFCWO)in the treatment of senile severe pneumonia.Methods:A total of 100 elderly patients with severe pneumonia admitted to the Intensive Care Department of Yulin Jingbian County People's Hospital from October 2020 to January 2024 were randomly divided into control group and observation group,with 50 cases in each group.The control group received fiberoptic alveolar lavage treatment on the basis of conventional treatment,and the observation group was combined with HFCWO treatment on the basis of the control group.The clinical efficacy,disappearance time of clinical symptoms and signs,blood gas index,serum C-reactive protein(CRP)and Procalcitonin(PCT)levels were compared between the two groups.Results:The total effective rate of observation group was higher than that of control group(P=0.037);the disappearance time of moist rales,fever,cough,heart failure and shortness of breath in observation group were shorter than those in control group(P<0.001);after treatment,the arterial carbon dioxide fraction was lower than before treatment,the arterial oxygen partial pressure and oxygenation index were higher than before treatment,and the observation group was better than the control group(P<0.05);after treatment,the serum CRP and PCT levels in two groups were lower than before treatment,and the observation group was lower than the control group(P<0.05).Conclusion:HFCWO is effective in the treatment of senile severe pneumonia,which can effectively improve clinical symptoms,regulate blood gas indexes and alleviate inflammatory response.
