经阴道二维超声下PI、S/D、RI在宫腔粘连诊断中的价值分析

Value analysis of PI,S/D,and RI in the Diagnosis of Uterine Adhesion under Transvaginal 2D Ultrasound

摘要 目的:探讨经阴道二维超声下动脉搏动指数(PI)、收缩期流速与舒张期流速比值(S/D)、阻力指数(RI)在宫腔粘连(IUA)诊断中的价值。方法:选取2021年10月—2023年10月渭南市临渭区妇幼保健计划生育服务中心收诊的40例IUA患者为观察对象(观察组),以同一时段接受妇科检查的40例健康体检者为对照组,均接受经阴道二维超声检查,对比两组患者以及观察组不同粘连程度患者的PI、S/D、RI参数。依据PI、S/D、RI参数单独及联合检验结果绘制受试者工作特征曲线,分析诊断价值。结果:观察组PI、S/D、RI参数高于对照组(P<0.001);重度粘连患者PI、S/D、RI参数高于轻、中度粘连患者,中度粘连患者PI、S/D、RI参数高于轻度粘连患者(P<0.05);PI、S/D、RI参数单独及联合诊断的曲线下面积值分别为0.889、0.892、0.928、0.958。结论:在IUA诊断及病情评估中,应用经阴道二维超声下PI、S/D、RI参数联合诊断,具有更高的可靠性。 Objective:To investigate the value of arterial pulsatility index(PI),systolic flow rate to diastolic flow rate ratio(S/D),and resistance index(RI)in the diagnosis of uterine adhesion(IUA)by transvaginal 2D ultrasound.Methods:Forty cases of IUA patients admitted to Weinan Linwei District Maternal and Child Health and Family Planning Service Center from October 2021 to October 2023 were selected as observation subjects(observation group).The forty healthy medical check-ups who received gynecological examination in the same time period were used as the control group,and all of them received transvaginal two-dimensional ultrasound examination.PI,S/D,and RI parameters were compared between the two groups of patients as well as between patients with different degrees of adhesion in the observation group.Based on the results of PI,S/D and RI parameters individually and in combination,ROC curves were drawn to analyze the diagnostic value.Results:PI,S/D,and RI parameters were higher in the observation group than in the control group(P<0.001);PI,S/D,and RI parameters were higher in patients with severe adhesions than in patients with mild and moderate adhesions,and PI,S/D,and RI parameters were higher in patients with moderate adhesions than in patients with mild adhesions(P<0.05);the area under the curve values of PI,S/D,and RI parameters for individual and combined diagnosis were 0.889,0.892,0.928,and 0.958,respectively.Conclusion:The application of combined diagnosis of PI,S/D,and RI parameters under transvaginal 2D ultrasound in the diagnosis and condition assessment of IUA has higher reliability.
