耐药型癫痫患者T淋巴细胞亚群和细胞因子与认知功能的相关性分析

Correlation analysis of T lymphocyte subsets and cytokines with cognitive function in patients with drug-resistant epilepsy

摘要 目的分析耐药型癫痫患者T淋巴细胞亚群CD4^(+)、CD8^(+)和细胞因子白细胞介素-1β(interleukin-1,IL-1β)、白细胞介素-6(interleukin-6,IL-6)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)与认知功能的相关性。方法选取2022年2月至2023年11月郑州大学第一附属医院收治的60例癫痫患者,据药物敏感性分为A组(耐药型)和B组(药物敏感型),每组30例,另选同期体检的30例健康居民为C组,均接受简易智力状态检查量表(mini-mental state examination,MMSE)、韦克斯勒记忆量表(Wechsler memory scale,WMS)及数字符号测试评估,同时检测其T淋巴细胞亚群及细胞因子水平,行Spearman相关性分析,分析耐药型癫痫患者T淋巴细胞亚群和细胞因子与整体智力、记忆力、注意力的相关性。结果A组、B组MMSE、WMS及数字符号测试分值显著高于C组(P<0.05);A组MMSE、WMS及数字符号测试分值显著高于B组、C组(P<0.05);A组、B组CD4^(+)、CD8^(+)显著低于C组,白细胞介素-1β(interleukin-1β,IL-1β)、白细胞介素-6(interleukin-6,IL-6)、肿瘤坏死因子α(tumor necrosis factor-α,TNF-α)显著高于C组(P<0.05);A组CD4^(+)、CD8^(+)显著低于B组、C组,IL-1β、IL-6、TNF-α显著高于C组(P<0.05);CD8^(+)与MMSE、WMS及数字符号测试分值呈正相关(P<0.05);IL-1β、IL-6、TNF-α与MMSE、WMS分值呈负相关(P<0.05)。结论耐药型癫痫患者T淋巴细胞亚群和细胞因子与认知功能存在显著相关性,其中CD8^(+)与患者认知功能呈正相关,IL-1β、IL-6、TNF-α与患者认知功能呈负相关,尽早对相关指标进行干预有助于预防认知功能障碍。 Objective Analysis of T lymphocyte subsets CD4^(+),CD8^(+),and cytokines interleukin-1(IL-1β),interleukin-6(IL-6),tumor necrosis factor-α(tumor necrosis factor-α,TNF-α)in patients with drug-resistant epilepsy Association with cognitive function.Methods According to drug sensitivity,60 patients with epilepsy in the First Affiliated Hospital of Zhengzhou University from February 2022 to November 2023 were divided into group A(drug-resistant type)and group B(drug-sensitive type),with 30 cases in each group.Another 30 healthy residents who came to the First Affiliated Hospital of Zhengzhou University for physical examination during the same period were selected as group C.All patients were assessed by mini-mental state examination(MMSE),Wechsler memory scale(WMS)and number symbol test,and their T lymphocyte subsets and cytokine levels were detected.Spearman correlation analysis was performed to analyze the correlation between T lymphocyte subsets and cytokines and the overall intelligence,memory and attention of patients with drug-resistant epilepsy.Results The test scores of MMSE,WMS and number symbol in groups A and B were significantly higher than those in group C(P<0.05).The test scores of MMSE,WMS and number symbol in group A were significantly higher than those in groups B and C(P<0.05).CD4^(+)and CD8^(+)in groups A and B were significantly lower than those in group C,and interleukin-1β(IL-1β),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)were significantly higher than those in group C(P<0.05).CD4^(+)and CD8^(+)in group A were significantly lower than those in groups B and C,and IL-1β,IL-6 and TNF-αwere significantly higher than those in group C(P<0.05).CD8^(+)was positively correlated with MMSE,WMS and number symbol test scores(P<0.05).IL-1β,IL-6 and TNF-αwere negatively correlated with MMSE and WMS scores(P<0.05).Conclusion T lymphocyte subsets and cytokines are significantly correlated with cognitive function in patients with drug-resistant epilepsy,among which CD8^(+)is positively correlated with cognitive function,and IL-1β,IL-6 and TNF-αare negatively correlated with cognitive function.Early intervention of relevant indicators is helpful to prevent cognitive dysfunction.
