乌梅丸联合美沙拉嗪治疗溃疡性结肠炎有效性研究

The effectiveness of Wumei Pills combined with mesalazine in treatment of ulcerative colitis

目的观察乌梅丸联合美沙拉嗪对溃疡性结肠炎(UC)患者的有效性。方法将我院60例UC患者随机分为研究组与对照组各30例。对照组给予美沙拉嗪治疗,研究组给予乌梅丸联合美沙拉嗪治疗。比较两组临床疗效,内镜Baron标准评分,发热、腹痛、腹泻、脓血便消失时间,中医证候积分,白介素-10(IL-10)、IL-4、IL-17、干扰素-γ(INF-γ),二胺氧化酶(DAO)、D-乳酸、内毒素含量,肠道菌群计数。结果治疗后,研究组总有效率高于对照组(P<0.05);两组Baron评分低于治疗前,研究组低于对照组(P<0.05);研究组发热、腹痛、腹泻、脓血便消失时间均短于对照组(P<0.05);两组中医证候积分,IL-17、INF-γ、DAO、D-乳酸、内毒素含量、肠球菌、大肠埃希菌指标均低于治疗前,研究组上述指标均低于对照组(P<0.05);两组IL-10、IL-4含量、双歧杆菌、乳酸杆菌计数均高于治疗前,研究组上述指标均高于对照组(P<0.05)。结论乌梅丸联合美沙拉嗪治疗UC患者疗效显著,促进西医和中医症状消失,改善内镜表现,降低炎症反应,平衡肠道菌群,保护肠黏膜屏障,调节Th1、Th2、Th17因子平衡状态,促进免疫功能恢复。

Objective To observe the efficacy ofWumei Pills combined with mesalazine in patients with ulcerative colitis(UC).Methods Sixty UC patients were randomly divided into a study group and a control group,with 30 cases in each group.Control group was treated with mesalazine,while study group was treated with Wumei Pills on the basis of control group.The therapeutic effects,Baron’s standard score for endoscopy,disappearance time of fever,abdominal pain,diarrhea,pus,and blood stool,TCM syndrome score,interleukin-10(IL-10),IL-4,IL-17,interferon-γ(INF-γ),diamine oxidase(DAO),Dlactate,endotoxin content,and gut microbiota count were compared between the two groups.Results After treatment,the total effective rate of study group was higher than that of control group(P<0.05);the Baron scores of both groups were lower than before treatment,and were lower in study group than control group(P<0.05);study group had shorter disappearing time of fever,abdominal pain,diarrhea,and bloody stool than control group(P<0.05);TCM syndrome scores,IL-17 and INF-γ,DAO,D-lactate,endotoxin content,Enterococcus and Escherichia coli indexes were lower than those before treatment in both groups,and were lower in study group than control group(P<0.05);the levels of IL-10 and IL-4,Bifidobacterium and Lactobacillus indexes in both groups were higher than before treatment,and were higher in study group than control group(P<0.05).Conclusion The combination of Wumei Pills and mesalazine has a significant therapeutic effect on UC patients,promoting the disappearance of Western and TCM symptoms,improving endoscopic manifestations,reducing inflammatory reactions,balancing gut microbiota,protecting intestinal mucosal barrier,regulating the balance of Th1,Th2,and Th17 factors,and promoting immune function recovery.
