蜈蚣败毒饮联合阿维A治疗寻常型银屑病疗效观察

Clinical observation on Wugong Baidu Decoction combined with Aviva A in treatment of psoriasis vulgaris

目的观察蜈蚣败毒饮联合阿维A治疗寻常型银屑病(PV)的疗效。方法将我院122例PV患者随机分为对照组与观察组各61例。对照组采用阿维A治疗,观察组采用蜈蚣败毒饮联合阿维A治疗。观察两组临床疗效,比较皮损面积严重指数(PASI评分)、瘙痒视觉模拟评分(VAS)、Zung焦虑自评量表(SAS)、血浆黏度、全血高切黏度、全血低切黏度、白介素-9(IL-9)、白介素-10(IL-10)、γ干扰素(IFN-γ)、白介素-4(IL-4)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)、纤维蛋白原(FIB)、D-二聚体(D-D)、纤溶酶原激活剂抑制物-1(PAI-1)、血管性假性血友病因子(vWF)。结果治疗后,观察组总有效率高于对照组(P<0.05);两组PASI、VAS、SAS评分低于治疗前,观察组上述指标低于对照组(P<0.05);两组血浆黏度、全血高切黏度、全血低切黏度低于治疗前,观察组上述指标低于对照组(P<0.05);两组IL-10水平高于治疗前,IL-9、IFN-γ、IL-4低于治疗前;观察组IL-10水平高于对照组,IL-9、IFN-γ、IL-4低于对照组(P<0.05);两组CD4^(+)、CD4^(+)/CD8^(+)高于治疗前,CD8^(+)低于治疗前;观察组CD4^(+)、CD4^(+)/CD8^(+)高于对照组,CD8^(+)低于对照组(P<0.05);两组FIB、DD、PAI-1、vWF水平低于治疗前,观察组上述指标低于对照组(P<0.05)。结论蜈蚣败毒饮联合阿维A提高治疗PV的疗效,减轻皮损、瘙痒、焦虑,可能的机制是改善血液流变学,纠正Th1/Th2失衡状态,抑制凝血。

Objective To observe the efficacy ofWugong Baidu Decoction combined with Aviva A in treatment of psoriasis vulgaris(PV).Methods A total of 122 PV patients were randomly divided into control group and observation group,with 61 cases in each group.Control group was treated with Aviva A,while observation group was treated withWugong Baidu Decoction on the basis of control group.The overall improvement rate,the severity index of skin lesion area(PASI score),Visual Analogue Scale of itching(VAS)score,Zung Self Rating Anxiety Scale(SAS)score,plasma viscosity,whole blood high shear viscosity,whole blood low shear viscosity,interleukin-9(IL-9),IL-10,interferon gamma(IFN-γ),IL-4,CD4^(+),CD8^(+),CD4^(+)/CD8^(+),fibrinogen(FIB),D-dimer(D-D),plasminogen activator inhibitor-1(PAI-1),and von Willebrand factor(vWF)of the two groups were observed and compared.Results After treatment,the overall effective rate of observation group was higher than that of control group(P<0.05);the PASI,VAS,and SAS scores of both groups were lower than before treatment,and were lower in observation group than control group(P<0.05);the plasma viscosity,whole blood high shear viscosity,and whole blood low shear viscosity of both groups were lower than before treatment,and were lower in observation group than control group(P<0.05);the levels of IL-10 in both groups were higher than before treatment,while IL-9,IFN-γ,and IL-4 were lower than before treatment,and the above indicators were superior in observation group to thoese in control group(P<0.05);the CD4^(+)and CD4^(+)/CD8^(+)levels in both groups were higher than before treatment,while CD8^(+)levels were lower,and the above indicators were superior in observation group to those in control group(P<0.05);the levels of FIB,D-D,PAI-1,and vWF in both groups were lower than before treatment,and were lower in observation group than control group(P<0.05).Conclusion Wugong Baidu Decoction combined with Aviva A improves the efficacy of treating PV,reduces skin lesions,itching,and anxiety.The possible mechanism is to improve hemorheology,correct Th1/Th2 imbalance,and inhibit coagulation.
