高血压患者用药依从性调查及影响因素分析

Analysis of Medication Adherence and influencing factors in Patients with Hypertensive

目的:调查高血压患者用药依从性调查并对影响因素进行分析。方法:选取福建省南平市建阳区徐市镇卫生院2020年1月—2024年1月收治的465例高血压患者,采用MMAS-8问卷对患者用药依从性进行调查,采用多元线性回归分析患者用药依从性的影响因素。结果:福建省南平市建阳区徐市镇卫生院465例高血压患者MMAS-8评分为3~8分,平均评分(5.41±1.07)分。单因素分析显示,文化程度、医保类型、合并病种、用药种类、不良反应与高血压患者用药依从性有关(P<0.05),且多因素分析显示,文化程度为本科以上(OR=2.789,95%CI:1.658~4.687,P<0.001)、医保类型为城镇医保(OR=2.462,95%CI:1.353~5.782,P<0.001)、合并病种0种(OR=3.645,95%CI:1.265~6.879,P<0.001)、用药种类≤2种(OR=2.978,95%CI:1.657~5.648,P<0.001)、不良反应无感觉或轻度(OR=2.142,95%CI:1.352~3.362,P<0.001)的高血压患者服药依从性较高。结论:福建省南平市建阳区徐市镇卫生院高血压患者用药依从性水平一般,文化程度、医保类型、合并病种、用药种类、不良反应为其用药依从性的主要影响因素,应针对这些影响因素采取有效措施实施干预。

Objective:To investigate the medication adherence of hypertensive patients and analyze the influencing factors.Methods:A total of 465 hypertensive patients admitted to the hospital from January 2020 to January 2024 were selected,and the MMAS-8 questionnaire was used to investigate the patients'medication compliance,and multiple linear regression was used to analyze the influencing factors of patients'medication compliance.Result:The MMAS-8 score of 465 hypertensive patients in a hospital was 3-8 scores,and the average score was(5.41±1.07)scores.Univariate analysis showed that education level,type of medical insurance,combined diseases,type of medication,and adverse reactions were related to medication compliance in patients with hypertension(P<0.05).Multivariate analysis showed that the education level was bachelor's degree or above(OR=2.789,95%CI:1.658-4.687,P<0.001),and the type of medical insurance was urban medical insurance(OR=2.462,95%CI:1.353-5.782,P<0.001),Patients with hypertension had higher medication compliance with 0 comorbidities(OR=3.645,95%CI:1.265-6.879,P<0.001),2 types≤of medication(OR=2.978,95%CI:1.657-5.648,P<0.001),and no sensation or mild adverse reactions(OR=2.142,95%CI:1.352-3.362,P<0.001).Conclusion:The level of medication compliance of patients with hypertension in hospitals is average,and the main influencing factors of medication compliance,such as education level,type of medical insurance,comorbid diseases,types of medication,and adverse reactions,should be taken to intervene in response to these influencing factors.
