摘要
目的:探究CT血管造影联合血清生长分化因子15(GDF-15)对冠脉狭窄程度预测价值及相关性。方法:选取2022年1月—2024年4月本院收治的110例冠状动脉心脏病(CHD)患者,根据冠状动脉硬化程度(Gensini)评分分为轻度组(n=45)和中重度组(n=65)。使用CT对所有CHD患者进行扫描检测,ELISA法检测GDF-15水平。对比不同冠状动脉硬化程度组CT血管造影检出率和血清GDF-15水平;ROC分析CT联合血清GDF-15检测对冠状动脉硬化程度预测价值;Spearman分析CT血管造影、血清GDF-15与冠脉狭窄程度相关性。结果:CT对轻度狭窄组检出符合率为88.89%,对中重度狭窄组检出符合率89.23%;中重度狭窄组GDF-15水平高于轻度狭窄组(P<0.05);ROC分析显示,CT联合血清GDF-15检测评估冠状动脉硬化程度线下面积为0.927,特异性(95.38%)和敏感度(91.11%)均较高;Spearman分析显示,CT血管造影、血清GDF-15与冠脉狭窄程度存在显著相关性(P均<0.05)。结论:CT血管造影对轻度、中重度狭窄患者检出符合率均较高,未检出原因可能与造影剂的使用有关;中重度狭窄组血清GDF-15水平显著高于轻度狭窄组;CT血管造影联合血清GDF-15检测对冠脉狭窄程度预测价值相对较高,且其之间存在一定的相关性。
Objective:To explore the predictive value and correlation of CT angiography combined with serum growth differentiation factor 15(GDF-15)on the degree of coronary artery stenosis.Methods:A total of 110 patients with coronary heart disease(CHD)admitted to our hospital from January 2022 to April 2024 were selected and divided into mild group(n=45)and moderate to severe group(n=65)according to Gensini score.All CHD patients were scanned by CT,and the level of GDF-15 was detected by ELISA.The detection rate of CT angiography and the level of serum GDF-15 in different coronary atherosclerosis groups were compared.ROC was used to analyze the predictive value of CT combined with serum GDF-15 detection for the degree of coronary atherosclerosis.Spearman analysis was used to analyze the correlation between CT angiography,serum GDF-15 and the degree of coronary stenosis.Results:The coincidence rate of CT in the mild stenosis group was 88.89%,and the coincidence rate of CT in the moderate and severe stenosis group was 89.23%.The level of GDF-15 in moderate to severe stenosis group was higher than that in mild stenosis group(P<0.05).ROC analysis showed that the area under the curve of CT combined with serum GDF-15in evaluating the degree of coronary atherosclerosis was 0.927,with high specificity(95.38%)and sensitivity(91.11%).Spearman analysis showed that CT angiography and serum GDF-15 were significantly correlated with the degree of coronary stenosis(P<0.05).Conclusion:The coincidence rate of CT angiography in patients with mild,moderate and severe stenosis is high,and the reason for non-detection may be related to the use of contrast agent.The serum GDF-15 level in the moderate to severe stenosis group was significantly higher than that in the mild stenosis group.CT angiography combined with serum GDF-15 detection has a relatively high predictive value for the degree of coronary stenosis,and there is a certain correlation between them.
