达格列净联合苯溴马隆治疗老年2型糖尿病合并痛风患者的效果

Effect of Dapagliflozin Combined with Benzbromarone in the Treatment of Elderly Patients with Type 2 Diabetes Mellitus Complicated with Gout

目的:探讨达格列净联合苯溴马隆治疗老年2型糖尿病(T2DM)合并痛风患者的临床疗效。方法:选取2021年9月—2023年9月在福建中医药大学附属第三人民医院收治的老年T2DM合并痛风患者70例,按照随机数表法分为苯溴马隆组与联合治疗组,各35例。苯溴马隆组给予常规治疗结合苯溴马隆,联合治疗组在此基础上应用达格列净,比较两组基线数据、T2DM和痛风症状改善情况、血糖、肾功能及炎症指标变化。结果:治疗4周后,联合治疗组T2DM症状改善和痛风状况改善总有效率高于苯溴马隆组,差异有统计学意义(P<0.05);联合治疗组空腹血糖、糖化血红蛋白、尿酸、尿素氮、白细胞介素-1β及肿瘤坏死因子-α优于苯溴马隆组(P<0.05)。结论:达格列净与苯溴马隆联合治疗可以更有效地控制老年T2DM合并痛风患者血糖与尿酸水平,缓解炎症,保护肾功能,从而提升临床疗效。

Objective:To probe into the clinical efficacy of dapagliflozin combined with benzbromarone in treatment of elderly patients with type 2 diabetes mellitus(T2DM)complicated with gout.Methods:A total of 70 elderly patients with T2DM complicated with gout admitted to the Third People's Hospital of Fujian University of Traditional Chinese Medicine from September 2021 to September 2023 were selected and divided into benzbromarone group and combined treatment group according to the random number table method,with 35 patients in each group.The benzbromarone group was given conventional treatment combined with benzbromarone,and the combined treatment group was given dapagliflozin on this basis.The baseline data,improvement of T2DM and gout symptoms,blood glucose,renal function and inflammation indexes were compared between the two groups.Results:After 4 weeks of treatment,the total effective rate of T2DM symptom improvement and gout improvement in the combined treatment group was higher than that in the benzbromarone group,and the difference was statistically significant(P<0.05).Fasting blood glucose,glycosylated hemoglobin,uric acid,urea nitrogen,interleukin-1βand tumor necrosis factor-αin the combined treatment group were better than those in the benzbromarone group(P<0.05).Conclusion:Dapagliflozin combined with benzbromarone can more effectively control the blood glucose and uric acid levels in elderly patients with T2DM and gout,relieve inflammation,protect renal function,and improve clinical efficacy.
